Evaluation of preoperative fear in elderly before elected surgery

Abstract:

Diplomová práce se věnuje problematice strachu a úzkosti u seniorů před plánovanou operací. Práce je rozdělena do dvou částí. První, teoretická část popisuje z obecného pohledu, co je to strach a úzkost, příčiny jejich vzniku a následné důsledky. V práci jsou představeny intervence k odstranění strachu a úzkosti a copingové metody, které pomáhají minimalizovat jejich dopad na mentální a fyzické zdraví. Cílem výzkumné části práce je zhodnotit předoperační strach a úzkost u seniora. Dále zjistit a porovnat hodnocení předoperačního strachu a úzkosti, v závislosti na věku, pohlaví, rodinném stavu, vzdělání, místě bydliště, v závislosti na typu operace, typu zvolené anestezie a na předchozích zkušenostech. Dalším cílem je zjistit nejčastější příčiny strachu a úzkosti seniora před operací. K dosažení cíle byl realizován kvantitativní výzkum, v němž byla data sbírána měřícími nástroji, a to pomocí standardizovaných dotazníků SAQ (The surgical anxiety questionnaire) a SFQ (Surgical Fear Questionnaire). Diplomová práce v závěru předkládá nejvýznamnější poznatky z vlastního výzkumu. Předoperační úzkost seniorů souvisí nejčastěji s obavami spojenými s chirurgickým výkonem (dřívější propuštění z léčení, absence nebo nedostatečnost informovanosti o tom, co je čeká), další příčinou je obava z neprobuzení se z operace, pochybení zdravotníků v postupech, riziko možných komplikací, které zapříčiní jejich zdravotní stav. Strach pociťují senioři nejvíce v oblasti strach z operace, strach z bolesti, strach z anestezie a následuje strach z vedlejších účinků. Z výsledků výzkumu dále plyne, že ženy mají ve srovnání s muži statisticky významně vyšší celkové skóre strachu, a celkové skóre předoperační úzkosti. V porovnání věkových kategorií 60-74 let a 75-89 let neexistuje statisticky významný rozdíl mezi skóre strachu a úzkosti. V oblasti předchozí zkušenosti byla prokázána závislost u všech případů pro seniory ze zkušeností špatnou, a dále se zkušeností žádnou. Závislost na dosaženém vzdělání nebyla prokázána pro žádné skóre, taktéž ani závislost na bydlišti podle počtu obyvatel neprokázala studie význam pro žádné skóre. Závislost na typu zvolené anestezie se prokázala nejvyšším skóre u strachu z celkové anestezie v 6 oblastech, druhé nejvyšší skóre bylo pro SAB ve 3 oblastech. Závislost na typu operace byla prokázána ve čtyřech případech a z toho plyne, že s rostoucí mírou operačního rizika je mírná závislost spojena s rostoucím skóre strachu z krátkodobých následků, strachu z dlouhodobých následků, celkové skóre strachu z operace a z obav z výkonu. Závislost strachu a úzkosti před operací v závislosti na rodinném stavu vyšel statisticky jako nevýznamný. V závěru práce předkládám doporučení pro praxi, zaměřené na edukaci, psychologickou podporu, copingové metody, a možnosti ohledně přípravy seniora k chirurgickému zákroku, které napomáhají minimalizovat předoperační strach a úzkost.

Abstract:

The diploma thesis deals with the issue of fear and anxiety in the elderly before a planned operation. The work is divided into two parts. The first, theoretical part describes from a general point of view what fear and anxiety are, the causes of their occurrence and subsequent consequences. The work presents interventions to eliminate fear and anxiety and coping methods that help minimize their impact on mental and physical health. The goal of the research part of the thesis is to evaluate preoperative fear and anxiety in the elderly. Furthermore, to find out and compare the assessment of pre-operative fear and anxiety, depending on age, gender, marital status, education, place of residence, depending on the type of operation, type of anesthesia chosen and previous experience. Another goal is to find out the most common causes of fear and anxiety of seniors before surgery. To achieve the goal, a quantitative research was carried out, in which data were collected with measuring instruments, using standardized questionnaires SAQ (The surgical anxiety questionnaire) and SFQ (Surgical Fear Questionnaire). At the end, the diploma thesis presents the most important findings from the research and appropriate recommendations to eliminate fear and anxiety. At the end, the diploma thesis presents the most significant findings from its own research. The pre-operative anxiety of senior sis most often related to fears connected with the surgical procedure (earlier discharge from treatment, absence or lack of information about what awaits them), another cause is the fear of not waking up from the operation, medical professionals errors in the risk of complications caused by their health condition. Seniors feel fear most in the area of fear of pain, fear of anesthesia, followed by fear of side effects. The results of the research show that women have a statistically significantly higher overal fear score and overal preoperative anxiety score compared to men. Comparing the age categories 60-74 years and 75-89 years, thereis no statistically significant difference between the fear and anxiety scores. In the area of previous experience, dependence was proven in all cases for seniors with poor experience, and further with no experience. Dependence on educational attainment was not demonstrated for any score, nor was dependence on residence according to the number of inhabitants significant for any score. Dependence on the type of anesthesia chosen was demonstrated by the highest score for fear of general anesthesia in 6 areas, the second highest score was for SAB in 3 areas. Dependence on the type of surgery was demonstrated in 4 cases, and iz follows that with increasing degree of operativerisk, moderate dependence is associated with increasing scores of fear of short-term consequences, fear of long-term consequences, total score of fear of surgery and fear of performance. The dependence of fear and anxiety before surgery on marital status was statistically insiqnificant. At the end of the thesis, present recommendations for practice, focused on education, psychological support, coping methods, and options for preparing the elderly for surgery, which help to minimalize preoperative fear and axiety.
