Choices while choosing sport activities for tetraplegic people in occupational therapy

Abstract:

Souhrn Název práce: Možnosti ergoterapie při výběru sportovní aktivity pro tetraplegika Cíl: Cílem práce je využití ergoterapie a její přínos při výběru některých sportů pro tetraplegika. Metodika: Má práce je rozdělena do dvou částí první část se zabývá teorií vybraného tématu, druhá je tvořena kazuistikou u klientky po spinálním poranění dg. spastická tetraplegie, transverzální léze míšní C6, stupeň tetraplegie 3, AIS (American Spinal Injury Association) A. Za účelem získání potřebných informací o klientce, jsem využila metodu: nastudování lékařské dokumentace se souhlasem klientky, rozhovoru. Ergoterapeutické vyšetření bylo prováděno za pomoci pozorování a standardizovaných (SCIM) i nestandardizovaných (LTPAQ SCI) testů. K celkovému vyšetření jsem využila následující metody: vyšetření aspekcí (držení, konfigurace, trofika), palpací (hybnost, tonus, svalová síla). V rámci terapie jsem využila nácviku ADL oblékání, soběstačnosti, mobility nácvik přesunů, protahovací a posilovací techniky. Také jsem využívala pomůcky jako Overball a Theraband. Výsledky: Výsledek ergoterapeutického působení vedl u klientky ke zlepšení v oblasti ADL oblékání a svlékání spodní poloviny těla, v mobilitě klientky při přesunech vozík lůžko, vozík automobil, v soběstačnosti naložení vozíku do automobilu. Klientka tyto aktivity zvládá po terapiích zcela samostatně. Došlo také k výběru vhodného sportu pro klientku lukostřelba. V tomto sportu proběhla edukace o přínosných cvičeních protahovacích i posilovacích. Proběhl výběr kompenzačních pomůcek. Zlepšila se také psychika klientky. Klientka získala motivaci k hledání další pohybové aktivity. Zvýšila se pohybová aktivita a doba volnočasových aktivit z původních 3,5 hodin týdně na 6,5 8,5 hodin týdně. Závěr: Napsání této práce mi rozšířilo znalosti v oboru ergoterapie ve specifickém odvětví a to ve sportu a pohybových aktivitách u tetraplegiků.

Abstract:

Summary Title of the thesis: Choices while choosing sport activities for tetraplegic people in occupational therapy Aim of the thesis: The aim of this thesis is the usage of occupational therapy and its benefit in the selection of certain sports for a tetraplegic. Methodology: My thesis is divided into two parts - the first one concerns the theory of the chosen topic, while the second is formed by case study of client after spinal injury - dg. spastic tetraplegia, transversal spinal lesion C6, degree of tetraplegia 3, AIS A. In order to gather necessary information about the client, I have chosen the following method: studying the medical documentation with the consent of the client and an interview. Occupational therapy inspection was made by observation and standardized (SCIM) as well as non-standardized (LTPAQ SCI) tests. For the purpose of total examination, I have used the following methods: exploration (posture, configuration, trophy), palpation (motility, muscle tone, muscle strength). Regarding the therapy, I practice ADL dressing, self-support, mobility practice of moving, stretching and strengthening techniques. Moreover, I have used tools such as Overball and Theraband. Results: The result of occupational therapy has led to an improvement of ADL - getting dressed and undressing the bottom part of the body, mobility of the client in cases of moving from the wheelchair to bed, from the wheelchair to the car, self-reliance - loading the wheelchair into the car. The client is able to do these activities fully self-sufficiently after the therapy. Also, a suitable sport was chosen for her - archery. At this sport, we educate auxiliary exercises of stretching and strengthening. Also, various compensation tools were chosen. The mental health of the client was also improved. She has found motivation to look for further physical activities. Her physical activity and the time spent with leisure activities has raised from 3,5 to 6,5 8,5 hours per week. Conclusion: By working out this thesis, I have broadened my knowledge in specific fields of occupational therapy, namely sports and physical activities of tetraplegics.
