Pain Management in the Postoperative Period - Knowledge and Experience of General Nurses

Abstract:

PORTER (roz. MACKOVÁ), Denisa. Management bolesti v pooperačním období - znalosti a zkušenosti všeobecných sester [disertační práce]. Ostravská univerzita v Ostravě, Lékařská fakulta, Ústav ošetřovatelství a porodní asistence. Školitel prof. PhDr. Andrea Pokorná, PhD. Studijní obor: P5341 Ošetřovatelství. Stupeň odborné kvalifikace: doktor filozofie (Philosophiae doctor, Ph.D.). Cíl: Hlavním cílem disertační práce je zhodnotit informovanost, znalosti ?a deklarované zkušenosti všeobecných sester v souvislosti s managementem pooperační bolesti v lůžkových zařízeních v České republice (ČR) a v Království Saúdské Arábie (KSA). Metodika: Kvantitativní, komparativní, prospektivní dotazníková studie - cohort survey study. Výběr souboru respondentů byl nepravděpodobnostní a účelový. Elektronická distribuce dotazníkových formulářů prostřednictvím nemocničních emailů a Oxford online survey. Sběr dat probíhal od července 2018 do prosince 2019. Výzkumným nástrojem pro sběr dat byl originální dotazníkový formulář (celkem 34 položek). Dotazníkový formulář se skládal ze dvou částí (demografická a explorativní část). Data byla statisticky zpracována v programu IBM SPSS 25 (hladina významnosti ? = 0,05). Výsledky a diskuze: Statistická analýza byla provedena z 833 (100 %) dotazníkových formulářů (n = 360 z ČR; n = 473 z KSA). Management pooperační bolesti se lišil v obou zemích v kontextu dodržování mezinárodních směrnic (guidelines) (p = 0,000). Rozdílné byly i názory na využívané škály k hodnocení bolesti na sledovaných odděleních (hodnota nabývala rozmezí p = 0,000 - 0,001). Léčba bolesti s využitím Patient Controlled Analgesia (dále jen PCA) se lišila mezi respondenty z obou zemí na sledovaných odděleních (hodnota nabývala rozmezí p = 0,000 - 0,009). Rozdíly byly v dostupnosti Acute Pain Service (Služba pro léčbu bolesti, dále jen APS) mezi oběma zeměmi na sledovaných odděleních (p = 0,000). Závěr: Ověřili jsme nejednotnost v managementu pooperační bolesti. Léčba ?s využitím PCA a spolupráce s APS byla častěji deklarována respondenty z KSA.

Degree of scientific qualification: philosophy doctor (Philosophiae doctor, Ph.D.). Aim: The main aim of the dissertation thesis is to evaluate the awareness, knowledge and declared experience of general nurses in connection with the management of postoperative pain, in inpatient facilities in the Czech Republic (CZ) and the Kingdom of Saudi Arabia (KSA). Methodology: Quantitative, comparative, prospective questionnaire study - cohort survey study. Non-probability and purposeful sampling was used when selecting the respondents. Electronic distribution of questionnaires was done via hospital emails and Oxford online survey. Data collection took place from July 2018 to December 2019. The research tool for data collection was the original questionnaire (a total of 34 items). The questionnaire consisted of two parts (demographic and exploratory part). Data were statistically processed in IBM SPSS 25 (significance level ? = 0.05). Results and Discussion: Statistical analysis was performed from 833 (100%) questionnaires (n = 36 from CZ; n = 473 from KSA). Postoperative pain management differed in both countries in the context of compliance with international guidelines ?(p = 0.000). Opinions differed on the scales used to assess pain in the specific departments (the value ranged from p = 0.000 - 0.001). Management of pain by using Patient Controlled Analgesia (PCA) differed between respondents from both countries in the specific departments (the value ranged from p = 0.000 - 0.009). Differences were on the availability of Acute Pain Service (APS) between two countries in the specific departments (p = 0.000). Conclusion: We verified inconsistencies in the management of postoperative pain. Treatment using PCA and cooperation with APS was more often declared by respondents from KSA.
