The reasons that lead women to choose home birth. Can the homelike environment be provided in hospitals during childbirth?

Abstract:

Předkládaná práce se zabývá problematikou domácích porodů. Hlavním cílem je zjistit důvody volby domácího porodu, na základě tohoto zjištění pak formulovat změny, které by se daly v praxi zavést a které by motivovaly ženu k porodu v nemocničním prostředí. Vedlejšími cíli je zjistit průběh domácího porodu, zmapovat hodnocení porodních zkušeností ženami, srovnat přístup porodních asistentek k porodu (pracujících v komunitě i nemocnici) a zjistit subjektivní hodnocení studia porodní asistence studentkami třetího ročníku. Nejčastějším důvodem volby domácího porodu je strach ze separace dítěte od matky a zásahy ze strany zdravotnického personálu. Tento problém lze zajistit přepracováním standardu péče o ženu a novorozence po fyziologickém spontánním porodu, který by umožnil nepřerušovaný kontakt mezi matkou a dítětem. Z přiložené ankety vyplývá, že se studentky cítí být připravené na výkon svého povolání, ale porodní asistentky konstatovaly, že mají nedostatky v oblasti edukace, komunikace a komunitní péče. Všechny nedostatky vyřčené studentkami i porodními asistentkami by mohly být podkladem pro změnu v přístupu vzdělávání porodních asistentek.

Abstract:

This thesis deals with issues of homebirth. The main aim of this work is to find out the reasons for the choice of homebirth and then formulate changes that could be intoduced in the practice and that would be motivation for woman to give birth in the hospital. The secondary aims are to find out the course of homebirth, to map the assessment of labor experience by womens and to compare the approach of midwives to birth (community and hospital workers), and to find out the subjective evaluation study of midwifery students of the third year of the study. The most common reason for choice of home delivery is the fear of separation of the child from the mother after birth and interventions by the midwife. This problem can be resolved by revising the standard of care for woman and a newborn after a normal spontaneous delivery which would enable uninterrupted contact between mother and child. The attached poll indicates that students feel ready to practice their profession, but midwives have found that they have shortcomings in education, communication and community care. All the shortcomings spoken by students and midwives could be the basis for a change in the approach of midwive's training.
