Importance of the molecular genetic diagnostics of CYP2D6 gene for clinical practice

Abstract:

Úvod. Hlavním trendem moderní farmakogenetiky je v současné době individualizace dávkování léčiv konkrétním pacientům, tzv. personalizace farmakoterapie. Jedněmi z nejvíce studovaných kandidátních genů jsou z této oblasti geny pro cytochromy P450, které metabolizují velkou část běžně užívaných léčiv. Kromě standardních léčiv s nízkým terapeutickým indexem, jako jsou cytostatika, některá antibiotika nebo imunosupresiva, sem řadíme i některá léčiva ze skupiny psychofarmak. Tato skupina, konkrétně antidepresiva, byla vybrána pro naši experimentální studii. Metody. V této studii jsme se zaměřili na farmakokinetiku antidepresiva paroxetinu a závislosti výskytu vedlejších účinků léčby na rychlosti jeho metabolizace na účinnou látku danou genotypem nebo fenotypem. U všech 98 pacientů zařazených do studie byla provedena molekulární analýza genu CYP2D6 a byl stanoven aktuální metabolický fenotyp. Dále byly posuzovány symptomy nežádoucích účinků léčby pomocí škál CGI1, UKU a ASEX. Všechna takto získaná data byla porovnána a byly stanoveny statistické korelace mezi jednotlivými faktory. Výsledky. Bylo zjištěno, že skupina pacientů s pomalým metabolismem má statisticky vyšší výskyt vedlejších účinků léčby na sexuální dysfunkce. Vyšší procento sexuálních dysfunkcí bylo pozorováno u žen. Dále byla dokázána souvislost mezi genotypem a fenotypem ve skupině pomalých metabolizátorů a souvislost stanoveného metabolického poměru s genotypem pacienta. Naopak nebyla dokázána souvislost mezi dávkou léčiva, věkem, pohlavím a genotypem ani fenotypem pacienta. Signifikantně delší byla pouze doba léčby u pomalých metabolizátorů. Taktéž nebyl dokázán vliv genotypu ani fenotypu na závažnost onemocnění danou škálou CGI1, na vedlejší účinky léčby dané škálou UKU ani na anxietu. Závěr. Studie dokázala, že farmakogenetický výzkum vztahu genotypu, fenotypu, metabolické aktivity a vedlejších účinků je v našich podmínkách proveditelný a že by podobně koncipované studie mohly v budoucnu sloužit pro lepší pochopení problematiky individualizovaného dávkování léčiv a usnadnit tak predikci rizika při preskripci léčiv s možnou lékovou interakcí na úrovni cytochromu P450, čímž by se snížila incidence nežádoucích účinků léčby. Zároveň ale studie poukázala na potřebu mnohem většího počtu zpracovávaných dat, než tomu bylo doposud.

Abstract:

Background. The main trend in modern pharmacogenetics is currently individualization of the drug dosing to individual patients, the personalization of pharmacotherapy. Ones of the most studied candidate genes from this region are genes for cytochrome P450 enzymes that metabolize a large group of the commonly used drugs. In addition to standard drugs with a narrow therapeutic index, such as antineoplastics, certain antibiotics or immunosuppressants, we rank here also some drugs from psychotherapeutics group. This group, namely antidepressants have been selected for our experimental study. Methods. We focused on the pharmacokinetics of antidepressant paroxetine and dependence of side-effects of paroxetine treatment on metabolism status in this study. The molecular genetics analysis of CYP2D6 gene has been performed and actual metabolic phenotype has been established in all 98 patients from this study. There were also symptoms of treatment adverse effects with scales CGI1, UKU and ASEX assessed. All those data have been compared and statistic correlations have been established between the all different factors. Results. We have found that the group of patients with slow metabolism shows statistically higher incidence of sexual dysfunction as treatment side effects. A higher percentage of sexual dysfunction was observed in group of women. Furthermore, the relationship between genotype and phenotype in a group of poor metabolizers has been proved. Also the relationship between metabolic ratio and metabolic genotype of the patient has been proved. On the other hand, there was not proved the relationship between dose, age, gender and genotype or phenotype of the patient. Just significantly longer length of treatment has been founded in group of poor metabolizers. There has not been demonstrated genotype or phenotype severity of the disease scale CGI1, the side effects on the UKU scale or anxiety. Conclusions. The study showed that pharmacogenetic research into the relationship between genotype, phenotype, metabolic activity and side effects is feasible in our conditions and that similarly designed study could serve in future much better understanding of the individualized drug dosing issue. Similar studies can facilitate the prediction of risk during drug prescribing and also show the potential of drug interactions at cytochrome P450 level, thereby it can help to reduce the incidence of adverse side effects of treatment. But results of this study have also highlighted the need for a much larger number of processed data than ever before.
