The home visiting service by a midwife for mothers in the postnatal period

Abstract:

Cíl: Cílem této výzkumné práce je zjistit míru informovanosti žen v oblasti péče o sebe samu a dítě a dále zjistit zkušenosti žen s péčí terénní porodní asistentky. Práce je zaměřena zejména na zmapování problémů a potřeb žen v období šestinedělí v rámci návštěvní služby porodní asistentky. Metodika: Pro sběr dat byla zvolena technika kvantitativního výzkumu prostřednictvím nestandardizovaného dotazníku, kdy výzkumný vzorek tvoří 165 žen. Veškeré otázky v dotazníku jsou uzavřené, ženy volí z možných odpovědí nebo hodnotí své subjektivní pocity pomocí pětistupňové Likertovy škály. Dotazníky byly rozdávány prostřednictvím komunitních porodních asistentek v období prosinec 2011 ? březen 2012 v rámci poslední návštěvy šestinedělky v domácím prostředí. Výsledky: Na základě výzkumu bylo zjištěno, že ženy jsou v oblasti míry informovanosti nejlépe seznámeny v rámci hospitalizace po porodu v oblasti péče hygieny rodidel a péče o porodní poranění, dále v péči o prsa a prevenci poranění bradavek, v oblasti výživy a životosprávy. Poněkud méně informací se ženám dostává v oblasti výskytu možných komplikací v šestinedělí a v péči o konečník a hemoroidy. Co se týče informovanosti žen v oblasti péče o dítě, z uvedených údajů je zřejmé, že nejlépe jsou ženy informovány v oblasti koupání dítěte, péče o jeho pokožku a krmení. Otázky týkající se převlékání dítěte a výměny plen a přípravy a ukládání dítěte ke spánku byly nejhůře hodnocené. Pro 77 % respondentek zlínského okresu je počet návštěv v poporodním období dostačující. Naprostá většina žen byla se službami terénních asistentek spokojená a hodnotí návštěvní službu v šestinedělí jako prospěšnou. V případě vlastní úhrady by 65,9 % šestinedělek služeb privátní porodní asistentky využilo. Také byl zjištěn statisticky významný rozdíl mezi ženami dle parity u problému techniky kojení, bolesti prsou, péče novorozence a psychické nerovnováhy. Zajímavým zjištěním byl fakt, že hierarchie potřeb u žen je odlišná od klasického dělení dle Maslowa. Jako nejvýznamnější byla respondentkami zhodnocena potřeba jistoty a bezpečí, spánku a odpočinku, kladné citové odezvy, potravy a tekutin a potřeba kvality prostředí. Potřeby biologické nemají před potřebami psychickými přednost, ale nacházejí se víceméně na stejné úrovni. Závěr: Z teoretických podkladů by se dalo předpokládat, že komunitní porodní asistentky mají v péči o matku a dítě nezastupitelné místo, ovšem v praxi narážejí na mnohá úskalí - zejména v oblasti administrativy, spolupráce s pojišťovnami a gynekology, ale také v míře informovanosti žen. Komunitní péče je specifická a výjimečná v možnosti holistického pohledu na člověka a dá se říct, že je ženě takzvaně ?šitá na míru?, což je zejména v období po porodu, kdy je citlivější a oslabená, velmi důležité a zároveň prospěšné. Abstract:

Goal: The aim of this thesis is to determine the degree of women awareness in self care and newborn care, as well as finding out their experience with outreach midwifery care. Main goal of this thesis is to map problems and needs of women in postpartum period within midwife services. Methodology: Data used in this thesis was collected using non-standardized questionnaire distributed to 165 women volunteers. All questions are closed, women have to choose answer from specified options or rate their subjective feelings using five-point Likert scale. Questionnaires were distributed through community midwives from December 2011 to April 2012 within their last visit in the postpartum period. Results: I have found out that women in the postpartum hospital health care are mostly informed about genital hygiene, taking care of birth injuries and breast and about prevention of nipple injuries. Also information connected with nutrition and diets are provided in sufficient amount. Less information is provided about the possibility of complications in the puerperium and care of the rectum and hemorrhoids. Regarding the collected newborn care data, most women are aware of bathing the newborn, its skin care and feeding. Worst rating was obtained in questions related to changing newborns? clothes and diapers and putting babies to bed. Furthermore, for 77 % of respondents from Zlin district the number of visits in the postpartum period is sufficient. Also most of the women were satisfied with services of field assistants and consider the home visiting service in the postpartum period as beneficial. Even if the midwifery services were provided as a private paid service, 65.9 % of women would pay for this service. Statistically significant differences were found between women (according to parity) in problems with breast-feeding techniques, breast pain, newborn care and mental misbalances. An interesting discovery is that women hierarchy of needs differs from the one presented by Maslow?s theory. According to women respondents? evaluation the most important needs are the need for certainty and safety, sleep and rest, positive emotional response, food and fluids and the need for ambience quality. Biological needs aren?t favoured to psychological needs but more or less are on the same level. Conclusion: On basis of the theoretical background, we assumed that community midwives are irreplaceable in the care of mother and child, but in practice they face many challenges - especially in administration, cooperation with insurance companies and gynecologists, but also with low women awareness. Community care is very specific and unique in the possibility of a holistic vision of man and we can say that it is "tailored" for every single woman, which is (mainly in the postpartum period when woman is more sensitive and weakened), very important and beneficial.
