The use of the objectifying classification and evaluation of consciousness of patients in intensive care

Abstract:

This thesis is focused on the issues of efficient use of objectivized classification and assessment of consciousness for patiens in intensive care units. Theoretical part explains the concept of consciousness, its characteristics, physiological and pathological changes of consciousness, algorithm of examination of patients with consciousness disorder, their needs and nursing care of these patients. Practical part was realized as a prospective clinical study within a sample of patiens. The main goal was to determine values of the chosen parameters connected with consciousness disorders for a sample of patients from intensive care unit in chosen hospitals and compare them with values noted in documentation. Our results show a significant difference between measured and documented values for a blood pressure only. This difference was predictable, since the delay between documented and our mesaurement was long enough the values of blood pressure to change (up to one hour). An interesting result for us was, that primary diagnosis is not related to the depth of consciousness disorder.

Abstract:

Diplomová práca je zameraná na problematiku využívania objektivizujúcej klasifikácie a hodnotenia vedomia u pacientov na jednotkách intenzívnej starostlivosti. Teoretická časť je zameraná na objasnenie pojmu vedomia, jeho vlastností, na priebeh stavmi vedomia, na fyziologické a patologické zmeny vedomia, algoritmus vyšetrenia pacientov s poruchou vedomia, potreby a ošetrovateľskú starostlivosť u týchto pacientov. Empirická časť bola realizovaná ako prospektívne klinické pozorovanie s vyšetrovacou intervenciou u konkrétnej skupiny. Hlavným cieľom prieskumu bolo zistiť sledované a dokumentované parametre a záznamy v súvislosti s poruchou vedomia u vybraného súboru pacientov na jednotkách intenzívnej starostlivosti v sledovaných nemocniciach a porovnať získané výsledky s výsledkami zaznamenanými v dokumentácii. Z výsledkov vyplynulo, že významný rozdiel medzi nameranými a dokumentovanými hodnotami bol len pri krvnom tlaku. Tento rozdiel sa dal predpokladať, pretože odstup medzi meraniami bol dosť dlhý (najviac jedna hodina) na to, aby sa hodnoty krvného tlaku zmenili. Pre nás zaujímavým výsledkom bolo to, že základná diagnóza nesúvisí s hĺbkou poruchy vedomia pacientov.
