Care of a Pregnant Woman with Rh Incompatibility from the Midwife's Perspective

Abstract:

Předložená bakalářská práce se zaměřuje na problematiku imunopatologického těhotenství, shrnuje odborné poznatky za posledních 10 let o různých typech serologických inkompatibilit mezi matkou a plodem. Teoretická část práce definuje imunopatologické těhotenství, klade důraz na inkompatibilitu matky a plodu v systému krevních skupin AB0 a v Rh systému, věnuje se také atypickým antigenům a protilátkám. V bakalářské práci jsou představené metody screeningu a diagnostiky izoimunizace. Dále se práce zabývá antenatální a postnatální profylaxí a její dostupností. V jednotlivých kapitolách jsou popsané mechanismy vzniku senzibilizace, izoimunizace, důraz je kladen na následky tohoto stavu pro novorozence. Samostatná kapitola je věnována hemolytické nemoci plodu a novorozence, metodám antenatální a postnatální diagnostiky a léčby a klinickým a laboratorním projevům. Praktická část práce prezentuje tři kazuistiky serologických inkompatibilit. Kazuistika týkající se Rh izoimunizace obsahuje podrobný popis průběhu těhotenství a porodu a dopady inkompatibility na novorozence. V této kazuistice je také popsána role porodní asistentky v péči o těhotnou ženu s Rh inkompatibilitou. Další dvě kazuistiky se zaměřují na dopady inkompatibilit na novorozence a podrobně popisují průběh jejich hospitalizaci. Závěr práce shrnuje poznatky o problematice a zdůrazňuje důležitost prenatálního screeningu, diagnostiky a profylaxe, edukace RhD negativních žen a kvalitní péče o těhotnou ženu a jejího novorozence.

Abstract:

The presented bachelor's thesis focuses on the issue of immunopathological pregnancy. It summarizes expert knowledge from the last 10 years on various types of serological incompatibilities between mother and fetus. The theoretical part of the thesis defines immunopathological pregnancy, emphasizes the incompatibility of mother and fetus in the AB0 blood group system and in the Rh system, and deals with atypical antigens and antibodies. The bachelor's thesis presents methods for screening and diagnosing isoimmunization. Furthermore, the thesis deals with antenatal and postnatal prophylaxis and its availability. The individual chapters describe the mechanisms of sensitization and isoimmunization, with emphasis on the consequences of this condition for the newborn. A separate chapter is devoted to hemolytic disease of the fetus and newborn, methods of antenatal and postnatal diagnostics and treatment, and clinical and laboratory manifestations. The practical part of the thesis presents three case studies of serological incompatibilities. The case study of Rh isoimmunization includes a detailed description of the course of pregnancy and childbirth and the effects of incompatibility on the newborn. This case study also describes the role of the midwife in caring for a pregnant woman with Rh incompatibility. The other two case studies focus on the effects of incompatibilities on newborns and describe in detail the course of their hospitalization. The conclusion of the thesis summarizes the knowledge on the issue and emphasizes the importance of prenatal screening, diagnostics and prophylaxis, education of RhD negative women and quality care for the pregnant woman and her newborn.
