Abstract:

Diplomová práce svým tématem omezovacích prostředků v intenzivní péči popisuje ve své teoretické části historii vzniku omezovacích prostředků a jejich evoluci napříč světem, a to zejména v oboru psychiatrie, jelikož tam se zpočátku nejvíce uplatňovaly. S modernizací zdravotnictví a vznikem pracovišť věnujícím pozornost intenzivní medicíně došlo vzhledem k charakteru hospitalizovaných pacientů k exodu omezovacích prostředků i do intenzivní péče, které je v této diplomové práci ponejvíce věnováno. Teorie poskytuje náhled do nejběžněji se vyskytující etiologie vzniku stavů vyžadujících použití jak farmakologických, tak mechanických omezovacích prostředků. Žádný mechanický omezovací prostředek však nelze uplatnit neregulovaně, proto se další kapitoly věnují legislativnímu vymezení problematiky, kompetencím a také definovaným typům omezovacích prostředků v České republice. Pro ucelení nabízí teoretická část diplomové práce srovnání schválených postupů při uplatnění omezovacích prostředků i mimo Českou republiku, a to konkrétně ve Spolkové republice Německo, na Slovensku a v USA. Výzkumná část diplomové práce sestává z kvalitativní studie zkoumající faktory ovlivňující používání omezovacích prostředků nelékařským zdravotnickým personálem v intenzivní péči. Na výzkumu participují respondenti z kategorie nelékařských zdravotnických pracovníků pracující v intenzivní péči a je veden formou rozhovorů s předem připravenými otázkami týkajícími se problematiky užívání omezovacích prostředků farmakologického i mechanického typu. Je kladen důraz na autenticitu výpovědí a nezkreslenost názorů respondentů, proto je přistoupeno ke kvalitativnímu zpracování studie. Výstupem práce je jednoduchý poster o čtyřech úrovních přístupu k pacientům rizikovým pro vznik zmatenosti a k pacientům o různé hladině zmatenosti. Poster velmi jednoduše znázorňuje zákonitosti vyplývající z uplatnění prevence vzniku zmatenosti pacienta, a to v případě situací, kdy již zmatenost u pacienta vznikla, nebo i v případě, kdy je nezbytné zavést omezovací prostředky u pacienta za účelem jeho rychlé deeskalace.

Abstract:

In the theoretical part, the thesis details the history of restraints and their evolution worldwide, particularly in the realm of psychiatry where they were initially applied the most. With modernisation of healthcare and the rise of specialized departments like intensive care units, restraints have increasingly been utilized in intensive care settings due to the specific needs of hospitalized patients, which is the central focus of this thesis. The theoretical framework provides insight into the most commonly encountered aetiology of conditions requiring the use of both pharmacological and mechanical restraints. However, it is important to note that the application of mechanical restraints should always be well-regulated. Therefore, the subsequent sections delve into the legislative frameworks defining the issue, necessary competencies, and approved types of restraints in the Czech Republic. To provide a comprehensive view, the theoretical portion includes a comparison of approved protocols for restraint application in other countries such as the Federal Republic of Germany, Slovakia, and the USA. The research segment of the thesis comprises a qualitative study aimed at investigating the factors influencing non-medical staff's use of restraints in intensive care settings. This study involves non-medical healthcare workers in intensive care and is conducted through interviews with structured questions pertaining to the use of both pharmacological and mechanical restraints. The research emphasizes the authenticity of responses and the neutrality of participants' viewpoints, hence adopting a qualitative methodology. The output of the work is then a simple poster outlining four levels of approach to patients at risk of developing confusion and those experiencing varying degrees of confusion. This visual tool simplifies the strategies for preventing and managing patient confusion, in situations where confusion has already occurred in the patient, and when it is necessary to introduce restraints in the patient to quickly de-escalate it.
