Abstract:

Cíl: Základním cílem práce bylo zjistit a popsat problémy, zkušenosti a potřeby rodinných příslušníků pečujících o dítě s život ohrožující nebo život limitující nemocí. Metodika: Zkoumání bylo realizováno kvalitativní metodou vzhledem k citlivosti zvoleného tématu. Sběr dat byl prováděn technikou nestandardizovaného rozhovoru s následnou analýzou obsahu dokumentů. Výběr účastníků rozhovorů byl záměrný. Celkový soubor tvořilo osm respondentů. Vztah k nemocnému dítěti byl v šesti případech jeden z rodičů, dále jeden prarodič a jeden sourozenec. Výsledky: Na základě analýzy dat bylo stanoveno šest základních kategorií: 1. začátek nemoci, 2. terapeutická fáze, 3. kontext zdravotní péče a podpory okolí, 4. přijetí života s nemocí, 5. umírání a smrt a 6. přijetí života bez dítěte. Výzkum ukázal, že dopad život ohrožujícího onemocnění dítěte, v některých případech spojených se smrtí, je na nejbližší pečující osoby obrovský a vždy s sebou nese těžký zásah do celého rodinného fungování. Výsledky potvrdily, že specializovaná paliativní péče je v našich podmínkách poskytována především onkologicky nemocným dětem a jejich rodinám. Rodiny s dětmi, které trpí neonkologickou život ohrožující nemocí, se s problémy spojenými s nemocí vyrovnávají sami. Těmto rodinám není k dispozici žádný podpůrný aparát a systém péče je u jejich dětí řešen nesystémově. Závěr: Mobilní hospicové služby a dětské lůžkové hospice mohou být vhodnou podporou pro obě skupiny dětí a jejich blízké. Hospicové sestry mohou pečujícím osobám pomoci zvládat nejen náročnou zdravotní péči o dítě, ale projít a překonat těžká období umírání, smrti a truchlení.

Abstract:

Objective: The main objective of this thesis was to identify and describe the challenges, experiences and needs of family members caring for a child with life-threatening or life-limiting illness. Methods: Due to the sensitivity of the chosen topic a qualitative research approach was implemented in this study. Data collection was carried out by a non-standardized interview method followed by a document content analysis. The selection of interviewed individuals was intentional. The sample consisted of eight respondents in total. Relationship to the sick child was in six cases one of the parents, one grandparent and one sibling. Results: Based on the analysis six basic categories were determined: 1. Beginning of the illness, 2. Therapeutic phase, 3. Context of health care and support environment, 4. Acceptance of life with illness, 5. Dying and death and 6. Acceptance of life without a child. The research has shown a life-threatening illness of a child that is in some cases associated with death of a child, has a tremendous impact on a caregiver and always heavily interferes with the functioning of the family. The results confirmed that in our circumstances a specialized palliative care is primarily provided to the oncologically ill children and their families. Families with children suffering from life-threatening, non oncological disease have to cope with the illness related issues alone. There is no support apparatus available for these families and care system of their children is being resolved non-systematically. Conclusion: Mobile hospice services and children´s inpatient hospice may be appropriate support for both groups of children as well as their relatives. Hospice nurses can help caregivers cope not only with demanding health care of their child, but also with going through and overcoming difficult periods of dying, death and grieving.
