Sense of coherence, mental health and selected personality characteristics in relation to postoperative adaptation of patients with chronic cardiovascular disease

Abstract:

Kardiovaskulární onemocnění je hlavní příčinou úmrtí ve všech zemích střední Evropy. Někteří pacienti v rámci léčby podstupují operaci srdce s mimotělním oběhem. Pooperační adaptaci těchto pacientů ovlivňují psychosociální faktory, díky nimž se každý jedinec adaptuje jinak úspěšně. Cílem diplomové práce bylo zjistit, zda na zmíněnou adaptaci má vliv smysl pro soudržnost, pozitivní duševní zdraví a vybrané osobnostní charakteristiky. Byl realizován kvantitativní výzkum, ve kterém byla pooperační adaptace operacionalizována pomocí dvou kritérií – pomocí délky hospitalizace měřené po operaci a pomocí kvality života půl roku po operaci. Pozitivní duševní zdraví pacienta bylo zjišťováno prostřednictvím metody Mental Health Continnum Scale – Short form, smysl pro soudržnost pomocí zkráceného Dotazníku orientace na život a pro diagnostiku osobnostních charakteristik byl použit Big Five Inventory (BFI-10). Jmenované diagnostické metody byly administrovány 3 dny před operací srdce. Pro zhodnocení pooperační kvality života byl půl roku po operaci poslán poštou dotazník Duke Health Profile. Výzkumný soubor se skládal ze 145 pacientů hospitalizovaných v Centru kardiovaskulární a transplantační chirurgie v Brně. Mnohonásobnou lineární regresí bylo zjištěno, že na délku pooperační hospitalizace má vliv pouze věk a otevřenost zkušenostem. Největším přínosem výzkumu je ověření, že pozitivní duševní zdraví dle Keyese má statisticky signifikantní vliv na pooperační kvalitu života pacienta. Před přidáním tohoto prediktoru se jevil statisticky významně i neuroticismus, ale jeho efekt se následně nepotvrdil. Závěry neukázaly existenci vlivu ostatních námi zkoumaných proměnných (smyslu pro soudržnost, extraverze a svědomitosti) na pooperační adaptaci.

Abstract:

Cardiovascular disease is the leading cause of death in all countries of Central Europe. Some patients within the treatment undergo cardiac surgery with extracorporeal circulation. The postoperative adaptation of these patients is influenced by psychosocial factors, which make each individual adapt differently. The aim of this thesis was to find out whether the sense of coherence, positive mental health and selected personal characteristics have an impact onto mentioned adaptation. Quantitative research was conducted in which postoperative adaptation was operationalised using two criteria - length of hospitalisation measured after surgery and quality of life half a year after surgery. The positive mental health of the patient was examined through the Mental Health Continnum Scale – Short form, sense of coherence by The Orientation of Life Questionnaire and Big Five Inventory (BFI-10) was used to reveal personality characteristics. These diagnostic methods were administered 3 days prior to cardiac surgery. To evaluate postoperative quality of life was used Duke Health Profile questionnaire which was sent by post six months after surgery. The research sample consisted of 145 patients hospitalized in the Center of Cardiovascular and Transplant Surgery in Brno. Multiple linear regression revealed that only age and openness to experience affect the length of postoperative hospitalisation. The greatest contribution of the research is verification that the positive mental health according to Keyes has a statistically significant effect on the postoperative quality of life of the patient. Before adding this predictor, neuroticism also appeared to be statistically significant, but the impact was not confirmed. In conclusion the existence of impact on postoperative adaptation by other variables we examined (sense of coherence, extraversion and conscientiousness) was not proven.
