System of patient-controlled analgesia in intensive postoperative care

Abstract:

Cíl: Cílem této práce bylo zjistit úroveň teoretických znalostí sester, pracujících na vybraných pracovištích pooperační intenzivní péče, o zásadách užití systému pacientem řízené analgezie (dále PCA). Dílčími cíli bylo zjistit, zda je užití systému PCA na zkoumaných pracovištích upraveno vnitřním předpisem, a dále porovnat úroveň vědomostí sester o problematice PCA v závislosti na těchto proměnných: na přítomnosti standardizace PCA na pracovišti respondentů, na absolvování specializačního studia v oboru ARIP (anestezie, resuscitace a intenzivní péče), na délce praxe respondentů v intenzivní péči a na způsobu proškolení respondentů v problematice PCA. Dále byly porovnány znalosti respondentů v rámci jednotlivých zdravotnických zařízení. Metodika: Metodou výzkumu bylo anonymní dotazníkové šetření. Vlastní distribuci dotazníků předcházel předvýzkum, jehož cílem bylo nalézt pracoviště splňující požadovaná kritéria. Zkoumaný soubor tvoří 86 respondentů, pracujících v době realizace výzkumu na odděleních pooperační intenzivní péče Fakultní nemocnice Ostrava, Fakultní nemocnice Olomouc, Nemocnice Prostějov a Nemocnice Šternberk. Sběr dat probíhal v období červen?říjen 2015. Pro základní popis souboru byla použita popisná statistika. Pro testování celkových znalostí byly použity neparametrické testy pro dva nebo více výběrů (Wilcoxonův test a Kruskal-Wallisův test). Pro hodnocení odpovědí dle jednotlivých otázek byl použit chí-kvadrát test, v případě nesplnění podmínek (nedostatečné počty) Fisherův exaktní test. Výsledky: V celkovém počtu získaných bodů za teoretické znalosti o problematice PCA se statisticky významně nelišily skupiny osob dle pracoviště s/bez standardizace systému PCA (p = 0,944), skupiny respondentů s/bez absolvování specializace ARIP (p = 0,389), skupiny s různou délkou praxe v intenzivní péči (p = 0,967), ani skupiny proškoleny různým způsobem (p = 0,355). Statisticky významný rozdíl mezi průměrným bodovým ziskem za správné odpovědi byl zjištěn ve skupinách respondentů v rámci jednotlivých zdravotnických zařízení (p = 0,004). Závěr: V České republice není metoda PCA příliš rozšířená. Bezpečné a účinné použití PCA je dáno několika faktory: vhodným výběrem pacienta, správným nastavením čerpadla, přizpůsobením dávek individuálním potřebám, průběžným monitoringem a účinnou edukací pacientů a jejich blízkých.

Abstract:

Aim: The aim of this work was to find out the level of theoretical knowledge of nurses working on chosen postoperative intensive care workplaces about the guidelines for the use of Patient-Controlled Analgesia system (PCA). Partial aims were to find out if the use of the PCA system on selected workplaces is governed by internal rules, to compare the knowledge level of the nurses about the PCA issues depending on these variables: the presence of PCA standardization on the respondent´s workplace, on completing studies specialized in the ARIP field (anesthesia, resuscitation, intensive care), on the lenght of respondent´s working experience in the intensive care and on the training method in the PCA issues. Also, the knowledge of respondents within each medical centre was compared. Methodics: The methodics of the research was an anonymous survey. The questionnaires followed a pilot study, the aim of which was to find a workplace which would meet the required criteria. The surveyed file consists of 86 respondents working in the departments of post-operative care in Ostrava University Hospital, Olomouc University Hospital, Prostějov Hospital and Šternberk Hospital during the research. The data collection took place during the period of June?October 2015. Descriptive statistics was used for the basic description of the file. Non-parametric tests for two or more selections (Wilcoxon test and Kurskal-Wallis test) were used for the general knowledge testing. Results: Comparing the total number of the points given for the theoretical knowledge about the PCA issues, groups of respondents did not differ significantly: groups of respondents according the workplace with/without PCA system standardization (p = 0,944), groups of respondents who passed/did not pass the ARIP specialization (p = 0,389), groups with a different length of working experience in the intensive care (p = 0,967) neither groups which were trained in a different way (p = 0,355). A significant difference between average points given for correct answers was found in the groups of respondents within each medical centre (p = 0,004). Conclusion: PCA method is not widely used in the Czech Republic. Safe and effective use of PCA is given by several factors: appropriate selection of the patient, correct setting of the pump, adjustment of the doses to individual needs, ongoing monitoring and effective education of the patients and their relatives.
