Genetic counseling in prenatal care

Abstract:

Cílem diplomové práce bylo zmapovat informovanost těhotných žen o prenatálním screeningu, z pohledu prvorodičky a druhorodičky. Dalším cílem bylo zjistit, které vyšetření těhotné ženy preferují při možnosti výběru. Dílčím cílem bylo zmapovat postoj a informovanost těhotné ženy podstupující invazivní výkon, neinvazivní test a ženy po porodu. Dalším dílčím cílem bylo zjistit, jaké jsou nejčastěji kladené otázky v průběhu genetické konzultace. Teoretická část se zabývá posunem prenatální diagnostiky, genetickým poradenstvím, zásadami dispenzární péče ve fyziologickém těhotenství, prováděním všeobecného screeningu, UZ vyšetřeními v jednotlivých trimestrech, invazivními i neinvazivními vyšetřeními a v neposlední řadě etickými a psychologickými aspekty žen. Výzkumná část se skládá z výzkumu kvalitativního a kvantitativního. Kvalitativní výzkum probíhal formou polostandardizovaného rozhovoru se dvěma těhotnými ženami, ženou po porodu a lékařkou. Rozhovor se zabýval problematikou informovanosti o prenatální péči a jejich postoji. Kvantitativní výzkum probíhal s využitím nestandardizovaného dotazníku, vytvořeného na základě výpovědi žen a lékařky. Dotazník byl rozdán těhotným ženám, přicházející na prvotrimestrální screening. Dotazník měl odhalit, jestli je rozdíl v informovanosti prvorodiček a druhorodiček a jaký je jejich postoj k jednotlivým vyšetřením.

Abstract:

The aim of this thesis was to investigate awareness of pregnant women about prenatal screening in terms of primiparas and secundiparas. Another goal was to determine which test pregnant women prefer. A secondary aim was to map the attitude and awareness of pregnant woman undergoing invasive procedures, non-invasive test and postpartum woman. Another objective was to find out what are the most frequently asked questions during genetic counseling. The theoretical part deals with the shift of prenatal diagnosis, genetic counseling, follow up care principles in physiological pregnancy, the implementation of screening ultrasound examination in each trimester, invasive and non-invasive examination, and last but not least, ethical and psychological aspects of women. The research part consists of qualitative and quantitative research. Qualitative research was conducted through semi-standardized interview with two pregnant women, postpartum woman and doctor. The interview dealt with the issue of awareness of prenatal care and their attitudes. Quantitative research was conducted using a special questionnaire, created on giving women a doctor. The questionnaire was distributed to pregnant women coming for first trimester screening. The questionnaire should reveal whether is the difference in awareness primiparas and secundiparas and what is their attitude to individual examination
