Hospital inpatient prospective payment system based on DRGs

Abstract:

Bakalářská práce se zabývá klasifikačním systémem DRG, který je ve světě využíván mimo jiné pro úhradu akutní lůžkové péče. Důraz je v práci kladen zejména na modifikaci IR-DRG, jenž se používá v České republice. Teoretická část se zabývá historii systému DRG a popisuje možné obměny tohoto systému, které se objevují v celosvětovém měřítku. Dále vysvětluje princip klasifikace pacientů v českém systému a postup, kterým probíhá úhrada za poskytnutou zdravotní péči. Rovněž zdůrazňuje možné negativní vlivy při používání tohoto systému a poskytuje stručný přehled o vybraných zahraničních modifikacích. V závěrečné části je detailněji rozebrán lineární regresní model, který je využíván v praktické aplikaci. Cílem praktické části této bakalářské práce je vytvořit model, který by vysvětloval náklady vybraných hospitalizačních případů, přičemž jsou použita anonymizovaná data hospitalizačních případů s hlavní diagnózou kýly. V praktické části bylo zjištěno, že výši nákladů hospitalizačních případů ovlivňují zejména typ operace, pobyt na jednotce intenzivní péče, věk pacienta a dále řada vedlejších diagnóz. Přínosem bakalářské práce je především ucelený pohled na systém DRG s důrazem na verzi používanou v České republice.

Abstract:

The thesis deals with DRG classification system, which is used worldwide, inter alia, for reimbursement of inpatient care. The main emphasis of this thesis was on modification of IR-DRG, which is currently used in the Czech Republic. The theoretical part deals with the history of the DRG system and describes the possible modifications of the system that occur worldwide. It also explains the principle of classification of patients in the Czech system and the processes by which reimbursement for the inpatient health care is provided. It also highlights the potential negative effects of using this system, and provides a brief overview of selected foreign modifications. In the final part linear regression model is introduced in detail, and further used in practical application. The main goal of the practical part of this thesis is to build up a model that would explain the hospitalization costs of selected cases, while using data of patients with hernia. In the practical part, it was found that the hospitalization costs are driven mainly by type of surgery, stay in the intensive care unit, the patient's age and a large number of secondary diagnoses. The benefit of this thesis lies primarily in a comprehensive view of the DRG system with emphasis on the version used in the Czech Republic.
