Study of the EGFR signaling pathway and microRNA expression profiles in response prediction to targeted therapy in colorectal cancer patients

Abstract:

Východisko: Terapeutické možnosti léčby metastatického kolorektálního karcinomu (mCRC) se rozšířily o anti-EGFR terapii (cetuximab, panitumumab), jež cílí a blokuje receptor pro epidermální růstový faktor (EGFR). Tato terapie je indikována jen pacientům s nemutovaným onkogenem KRAS (wt-KRAS), který je podmínkou pro úspěšnou léčbu. I přesto je klinický benefit léčby pozorován jen u přibližně pětiny pacientů s wt-KRAS. Z tohoto důvodu je nutné nalézt nové biomarkery schopné predikovat odpověď k anti-EGFR terapii, která v případě neúčinnosti zbytečně oddaluje možnost podání jiné potenciálně efektivní terapie. Těmito biomarkery by mohly být miRNA, jež jsou zapojeny do klíčových signálních drah a regulují také jednotlivé složky signální dráhy EGFR. Metody: Ze vzorků nádorové tkáně odebraných před zahájením léčby cetuximabem byla vyizolována celková RNA obohacená o frakci krátkých RNA, která byla použita pro stanovení expresních profilů miRNA pomocí dvou různých technologií. Nakonec byla provedena validace a analýza dat za účelem nalezení miRNA schopných předpovědět odpověď k anti-EGFR terapii. Výsledky: V této studii byly identifikovány miRNA s rozdílnou expresí mezi pacienty odpovídajícími a neodpovídajícími na léčbu cetuximabem. Současně byl sestaven klasifikátor, který na základě rozdílné exprese miRNA dokázal rozlišit tyto dvě skupiny pacientů s vysokou senzitivitou i specificitou. Následná validace potvrdila korelaci mezi hladinami vybraných miRNA s dobou do progrese a celkovým přežitím. Závěr: Na základě získaných výsledků se zdá, že by miRNA mohly sloužit jako nové prediktivní biomarkery odpovědi na anti-EGFR léčbu u mCRC pacientů s wt-KRAS, což by u nich přispělo k vyšší individualizaci léčby a lepší kvalitě života. Další studium těchto molekul v souvislosti se signální dráhou EGFR by pak mohlo přispět k detailnímu pochopení molekulárních mechanizmů rezistence.

Abstract:

Introduction: Anti-EGFR therapy, which targets and blocks the epidermal growth factor receptor (EGFR) has extended therapeutic options for metastatic colorectal cancer (mCRC) treatment. This therapy is indicated only to patients with wild-type oncogene KRAS (wt-KRAS), because it is a condition for successful treatment. Even so, the clinical benefit of the therapy is observed only in approximately one-fifth of patients with wt-KRAS. Hence, it is necessary to find new predictive biomarkers that would enable to predict therapeutic response to anti-EGFR therapy. It seems miRNAs may serve as predictive biomarkers, because they are involved in key oncogenic pathways and they also regulate key components of EGFR signaling pathway. Methods: Total RNA enriched with small RNAs from tumor samples resected before starting cetuximab treatment was isolated and subsequently used for the determination of miRNAs expression profiles using two different technologies. Finally, the validation and data analysis was performed to identify predictive miRNAs for anti-EGFR therapy. Results: In this study, miRNAs with significantly different expression profiles between responders and non-responders to cetuximab treatment were determined. Simultaneously, the classifier based on differentially expressed miRNAs was developed. It was able to distinguish these two groups of patients with high sensitivity and specificity. Following validation confirmed the correlation between expression levels of selected miRNAs with time to progression and overall survival. Conclusion: Based on the obtained results, it seems miRNAs may serve as new predictive biomarkers of anti-EGFR treatment response in mCRC patients with wt-KRAS. Thus, they introduction could lead to higher level of individualized therapy and better quality of life in these patients. Further study of these molecules in the context of EGFR signaling pathway could contribute to a better understanding of molecular mechanisms of resistance.
