Endometriosis in the context of family planning and pregnancy

Abstract:

Cíl: Cílem práce bylo zjistit a popsat, jaké problémy se vyskytují u žen s diagnózou endometrióza. Dále zaměřit se na somatickou, psychickou a sociální oblast, v kontextu těhotenství a porodu. Metodika: Ve výzkumném šetření byla použita metoda kvalitativního výzkumu. Výzkumný soubor tvořilo záměrně vybraných osm žen s diagnózou endometrióza, ženy rodily v perinatologickém centru. Jako metoda sběru dat byla použita forma individuálního polostrukturovaného rozhovoru. Výsledky: Na základě analýzy dat byly stanoveny kategorie a podkategorie. Analýzou rozhovorů bylo zjištěno, že dysmenorea, dyspareunie a neplodnost byly nejčastějším symptomem onemocnění. Je evidentní, že všechny ženy bolestivou menstruaci považovaly za obvyklý průběh menstruačního cyklu. Ženám nebyly nabídnuty alternativní metody ke zmírnění pánevních bolestí. Dále bylo zjištěno, že pacientky postrádaly informace o příznacích, průběhu nemoci a možných komplikacích, které mohou u porodu nastat. Závěr: Ženy prožívaly nápor negativních emocí, ale také pociťovaly úlevu a radost. Jejich největší oporou byl partner. Problémy spojené s endometriózou jsou intimní a některé ženy mají ostych před lékaři. Proto jsou zde důležité komunitní porodní asistentky, které se dokáží vcítit do obtíží nemocných žen a nadále s nimi udržovat kontakt v další péči či následujícím těhotenství.

Abstract:

Aim: The aim of the diploma thesis was to identify and describe problems caused by endometriosis and to focus on somatic, psychological and social area in the context of pregnancy and childbirth. Methods: The survey was processed by qualitative research. The study group consisted of eight women with the diagnosis of endometriosis, who delivered in perinatology center Olomouc. The data was collected using individual semi-structured interview. Results: After analysing the data, we defined categories and subcategories. The most common symptoms of the disease were dysmenorrhea, dyspareunia and infertility. It was evident that dysmenorrhea was considered as a usual consequence of the menstrual cycle by all women. Alternative methods to relieve pelvic pain were not offered to women in the study group. It was also found that patients lacked information about the symptoms of the disease, and about possible complications that may occur during childbirth. Conclusion: Women experienced rush of negative emotions, but also felt relief and joy. Their biggest support was their partner. Problems associated with endometriosis are intimate and some women are ashamed to inform their doctor. The role of community midwives is very important, because they empathize with the difficulties of women with endometriosis, and support them in future life and family planning.
