Post mortem analysis of insulin and its selected analogues from blood by liquid chromatography with tandem mass spectrometry detection

Abstract:

Tématem bakalářské práce byl vývoj metody pro stanovení insulinu a jeho vybraných analogů (aspart, lispro, glulisin, glargin, detemir a humánní insulin) z krve u post mortem paceintů. Analýza probíhala na Oddělení klinické farmakologie, Ústavu laboratorní medicíny ve Fakultní nemocnici Ostrava pomocí kapalinové chromatografie s tandemovou hmotnostní detekcí. Teoretická část této práce byla věnována insulinu a jeho analogům, syntéze a funkcím insulinu, dále také onemocnění diabetes mellitus a jeho léčbě. Následuje kapitola úprava biologické matrice, chromatografické metody a hmotnostní spektrometrie. Poznatky z těchto kapitol byly uplatněny v experimentální části. Vývoj metody probíhal na kapalinovém chromatografu Acquity H-class Plus (kvartérní systém) ve spojení s hmotnostním detektorem Xevo TQ-S cronos (QqQ detektor, trojitý kvadrupól) od společnosti Waters. Separace látek probíhala na koloně Cortecs UPLC C18+ 2,1x100 mm o velikosti částic 1,6 ?m s předkolonou Cortecs UPLC C18+ VanGuard o velikosti částic 1,6 ?m od společnosti Waters. Vzorky byly ionizovány pomocí elektrospreje v pozitivním modu (ESI+). Doba chromatografické analýzy byla 5 minut při gradientové eluci. Vzorky krve byly upravovány pomocí metody srážení proteinů (PPT), následovala mikroextrakce na pevné fázi (?SPE). Během vývoje bylo testováno různé složení mobilní fáze a nastavení SIR (sledování vybraného iontu) a MRM (sledování několika fragmentací) přechodů. Linearita byla ověřena kalibrační křivkou. Hodnoty korelačního koeficientu pro všechna insulinová analoga byly větší než 0,99.

Abstract:

The topic of the bachelor thesis was the development of a method for the determination of insulin and its selected insulin analogues (aspart, lispro, glulisin, glargine, detemir and human insulin) from post mortem blood patients. The analysis was performed of the department of Clinical Pharmacology, Institute of Laboratory Medcine, at the University Hospital Ostrava by liquid chromatography with tandem mass spectrometry detection. The theoretical part of this thesis was focused on insulin and its analogues, synthesis and functions of insulin, as well as diabetes mellitus and its treatment. This is followed by a chapter on sample preparation of biological matrix, chromatographic methods and mass spectrometry. The knowledge from these chapters was applied in the experimental part. The method development was carried out on an Acquity H-class Plus liquid chromatograph (quaternary system) with an Xevo TQ-S cronos mass detector (QqQ detector, triple quadrupole) from Waters. Separation of the compounds was performed on a Cortecs UPLC C18+ 2.1x100 mm 1.6 ?m particle size column with a Cortecs UPLC C18+ VanGuard pre-column 1.6 ?m particle size from Waters. Samples were ionized using electrospray in positive mode (ESI+). The time of chromatographic analysis was 5 minutes at gradient elution. Blood samples were processed using the protein precipitation method (PPT), followed by solid-phase microextraction (?SPE). During method development was used different type of mobile phase compositions and setting of mass spectrometr scan for SIR (selected ion monitoring) and MRM (multiple reaction monitoring) transitions were tested. The linearity was verified by a calibration curve. The correlation coefficient values for all insulin analogues were greater than 0.99.
