Time trends in acute inpatient care in the Czech Republic

Abstract:

A key aspect of the complex and ever-developing system, that is the healthcare sector, is rational planning, effective management and timely decision-making based on up-to-date information. A good indicator of the distribution and consumption of health care is, among others, the structure of acute inpatient care and its development in time. In this bachelor's thesis, we will therefore focus on creating models, that are not only able to capture and explain the development of trends in acute inpatient care but can be also utilized to forecast its future developments. The theoretical part of the thesis deals with the issue of classification of acute inpatient care in the Czech Republic, focusing on the classification system CZ-DRG and including research of statistical methods aimed at regression analysis of count data and time series analysis. In the practical part of the thesis, those methods are applied to aggregate data of the National Register of Reimbursed Health Services. The outcome of the thesis lays out several types of models, which describe the structure of acute inpatient care on various levels depending on long-term trends and seasonality. The models are afterwards compared with one another based on their ability to correctly forecast future developments.

Abstract:

Klíčovým aspektem fungování tak komplexního a soustavně se vyvíjejícího systému, jakým je zdravotní sektor, je racionální plánování, efektivní hospodaření a včasné přijímání rozhodnutí založené na aktuálních poznatcích. Dobrým ukazatelem distribuce a spotřeby zdravotní péče je například struktura akutní lůžkové péče a její vývoj v čase. V této bakalářské práci se proto věnujeme tvorbě modelů, které dokážou trendy v akutní lůžkové péči nejen zachytit a vysvětlit, ale i využít k předpovědi jejího budoucího vývoje. Teoretická část práce popisuje problematiku klasifikace akutní lůžkové péče v České republice se zaměřením na klasifikační systém CZ-DRG a obsahuje rešerši statistických metod zacílenou na regresní analýzu diskrétních dat a analýzu časových řad. V praktické části jsou tyto metody aplikované na agregovaná data Národního registru hrazených zdravotních služeb. Výsledkem práce je několik typů modelů, které popisují strukturu akutní lůžkové péče na různých úrovních z hlediska dlouhodobých trendů a sezónnosti. Nakonec jsou mezi sebou jednotlivé modely srovnané na základě jejich schopnosti budoucí vývoj struktury akutní lůžkové péče správně předpovídat.
