Physiotherapy in recurrent ankle joint distortions in basketball

Abstract:

Úvod: Bakalářská práce se zabývá problematikou opakovaných distorzí hlezenního kloubu v basketbalu. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta s opakovanými distorzemi hlezenního kloubu v basketbalu pomocí vhodných fyzioterapeutických metod, postupů a konceptů. Metodika: Hlavní neboli praktická část je kazuistika pacientky po opakovaných distorzích levého hlezenního kloubu. První byla odebrána anamnéza, po které následovalo vyšetření vstupního kineziologického rozboru, na základě kterého byl stanoven krátkodobý fyzioterapeutický plán. Pacientka absolvovala celkem deset společných terapií, které se uskutečnily v časovém období osmi týdnů. K naplnění cílů krátkodobého plánu byly využity mobilizační techniky, postizometrická relaxace, technika měkkých tkání, postfacilitační strečink, cvičení s Thera-Bandem dle Brüggerova konceptu, senzomotorická stimulace, Flowin?, propriofoot, proprioceptivní neuromuskulární facilitace, akrální koaktivační terapie a dynamická neuromuskulární stabilizace. Při závěrečné terapii byl proveden výstupní kineziologický rozbor a dle jeho výsledků byl stanoven dlouhodobý fyzioterapeutický plán. Výsledky: Po absolvování celé fyzioterapeutické intervence došlo u pacientky ke zlepšení aktivního rozsahu pohybu do dorzální flexe z původního základního postavení o 5°. Došlo také k upravení stereotypu chůze a zlepšení stability zejména hlezenních kloubů. Pozorovat můžeme i progres v celkovém držení těla, mírné zvětšení některých hodnot obvodů na levé dolní končetině, obnovení správné svalové délky či obnovení kloubní vůle a posunlivosti fascií. Závěry: Vzhledem k dosaženým výsledkům a subjektivnímu hodnocení pacientky lze usoudit, že použité metody během terapií byly vhodné a pomohly ke zlepšení stavu.

Abstract:

Introduction: The bachelor´s thesis deals with the problem of recurrent distortions of the ankle joint in basketball. The aim: The aim of the bachelor´s thesis is to contribute to the solution of the problem of physiotherapy in a patient with recurrent ankle joint distortions in basketball by using physiotherapeutic methods, procedures and concepts. Methodology: The main or practical part is a case report of a patient after reccurent distortions of the left ankle joint. At first medical history was taken, followed by an initial kinesiological analysis, on which base was a short-term physiotherapy plan determined. The patient underwent a total of ten physiotherapy sessions over a time period of eight weeks. Mobilization techniques, post-isometric relaxation, soft tissue mobilization, post-facilitation stretching, Thera-Band exercises based on the Brügger concept, sensomotory stimulation, Flowin?, propriofoot, proprioceptive neuromuscular facilitation, acral coactivation therapy and dynamic neuromuscular stabilization were used to achieve the goals of the short-term plan. During the final therapy, an outcome kinesiological analysis was completed and a long-term physiotherapy plan was determined according to the results. Results: After completing the entire physiotherapy intervention, the patient improved her active range of motion into dorsiflexion by 5° from the original baseline position. There was also a positive change in walking stereotype and an improvement in stability, especially in ankle joints. We can also observe progress in the overall posture, a slight increase in some values of girth on the left lower limb, restoration of correct muscle length or restoration of joint mobility and fascia movement. Conclusions: Considering achieved results and patient´s subjective evaluation, it can be concluded that the used methods during the therapies, were appropriate and lead to the improvement of patient´s condition.
