Validation Study of Phonemic Verbal Fluency Test for Assessment of Cognitive Deficit in Patients with Amnestic Mild Cognitive Impairment and Alzheimer's Disease

Abstract:

Kognitivní deficit a rozvíjející se dysexekutivní deficit je typickým znakem jak pro pacienty s amnestickou mírnou kognitivní poruchou (aMCI), tak pro pacienty s Alzheimerovou demencí, a proto je důležité vyvíjet neuropsychologické diagnostické nástroje, které budou schopny chorobu detekovat již v časném stadiu. Hlavním cílem studie bylo ověření citlivosti a psychometrických vlastností české verze testu fonematické verbální fluence na skupině pacientů s aMCI a Alzheimerovou chorobou. Výzkumný soubor tvořilo celkem 71 respondentů: 29 respondentů tvořilo kontrolní skupinu neurologicky a kognitivně zdravých jedinců a 42 tvoří klinickou skupinu pacientů s aMCI a Alzheimerovou chorobou. Konkrétně byla ověřována rozlišovací schopnost skóru celkový počet slov, počet switchů a průměrná velikost clusteru. Ukázalo se, že celkový počet slov skupinu zdravých jedinců a skupinu pacientů s aMCI i Alzheimerovou chorobou. Počet switchů byl diskriminující jen mezi skupinou zdravých jedinců a pacientů s Alzheimerovou chorobou. Průměrná velikost clusteru opět významně rozlišovala obě skupiny pacientů od skupiny zdravých jedinců. Pro signifikantní rozdíly mezi skupinami ve vybraných skórech je provedena ROC (Receiver Operating Characteristic) analýza ve snaze stanovit optimální diagnostické a screeningové cut-off skóry.

Abstract:

Cognitive impairment is a common feature in patients with amnestic mild cognitive impairment (aMCI) and also in patients with Alzheimer´s disease, and hence the development of neuropsychological diagnostic methods fit for its early detection is needed. The main purpose of our research study is the validation of sensitivity and psychometric properties of the Czech version of phonemic verbal fluency test in the assessment of cognitive impairment in patients with amnestic mild cognitive impairment (aMCI) and also in patients with Alzheimer´s disease. The phonemic version of verbal fluency test was administred to sample group of 71 subjects: there was 29 healthy subjects in control group and 42 subjects in patient group with either aMCI or Alzheimer´s disease. Namely, discriminant validity of total number of words generated, number of switches and mean cluster size scores was investigated. The results show that total number of words generated score significantly differs healthy subjects from both aMCI patient group and Alzheimer´s disease patient group. The number of switches significantly differs healthy subjects from aMCI patient group but not from Alzheimer´s disease patient group. The mean cluster size score also significantly differs healthy subjects from both aMCI patient group and Alzheimer´s disease patient group. In order to determine optimal diagnostic and screening cut-off scores, ROC (Receiver Operating Characteristic) analysis is used.
