Preimplantation examination of unbalanced chromosomal aberrations using the NGS method

Abstract:

Bakalářská práce se zabývá problematikou nebalancovaných chromozomových aberací u embryí, jejichž rodiče jsou nositeli reciprokých či robertsonských balancovaných translokací. Nositelé těchto translokací mají často problémy s reprodukcí, které jsou spjaty s nebalancovanými formami familiárních translokací u jejich potomků. Výzkumný soubor tvořilo 383 úspěšně vyšetřených embryí pocházejících od nosičů translokací. Nebalancované aberace byly vyšetřovány z odebraných buněk trofoektodermu a detekovány pomocí speciálně upravené molekulární metody sekvenování nové generace (NGS) v rámci preimplantačního vyšetření strukturálních přestaveb (PGT-SR). V práci jsem chtěla ověřit, že nosiči reciprokých balancovaných translokací mají vyšší podíl embryí s nebalancovanými formami translokace než nosiči robertsonských translokací. Podíl nebalancovaných forem familiárních translokací u embryí od nositelů reciprokých translokací byl téměř dvakrát vyšší než u embryí od nositelů robertsonských translokací (53,6 % vs 28,4 %). Statisticky signifikantně vyšší podíl nebalancovaných embryí od nositelů reciprokých translokací byl potvrzen pomocí Fisherova exaktního testu (p < 0,00001). Muži i ženy nesoucí jakýkoli z obou typů translokace mají téměř stejný podíl nebalancovaných embryí, na pohlaví nosiče translokace tudíž nezáleží. Při PGT-SR můžeme zachytit mnoho různých meiotických segregačních vzorů, některé z nich byly v práci zdokumentovány. Kvůli aneuploidním změnám vzniklým de novo nemohlo být doporučeno k transferu 26,9 % úspěšně vyšetřených, v rámci PGT-SR balancovaných, embryí.

Abstract:

This bachelor's thesis addresses the issue of unbalanced chromosomal aberration of embryos, whose parents are carriers of balanced either reciprocal or Robertsonian translocations. Carriers of these translocations often experience reproduction problems, which are related to unbalanced forms of the familiar translocations in their offspring. The research cohort consisted of 383 successfully examined embryos originating from carriers of the translocations. Unbalanced aberrations were examined from trophectoderm cells using a special modification of new generation sequencing (NGS) molecular method, in frame of preimplantation genetic testing for structural chromosomal rearrangements (PGT-SR). The aim of my thesis was to verify the hypothesis that carriers of balanced reciprocal translocations have a higher proportion of embryos with unbalanced forms of the translocation than carriers of Robertsonian translocations. The percentage of unbalanced forms of familiar translocations in embryos of carriers of reciprocal translocations was nearly double than in embryos of carriers of Robertsonian translocations (53.6 % vs 28.4 %). Statistical significance of the higher proportion of unbalanced embryos in carriers of reciprocal translocations was confirmed using Fisher's exact test (p < 0.00001). Men and women carrying any of both translocation types have nearly the same proportion of unbalanced embryos, therefore the gender of carrier is not significant. Many different meiotic segregation patterns may be observed within PGT-SR, some of them were documented in the thesis. 26.9 % of successfully examined embryos with balanced results within PGT-SR could not be recommended for embryotransfer due to aneuploidies arisen de novo.
