Physiotherapy for fractures in the proximal humerus

Abstract:

Úvod: V bakalářské práci, jsem se zabývala fyzioterapií u probanda se zlomeninou proximální části humeru. Cíl práce: Cílem práce je přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u vybraného klienta po zlomenině v oblasti proximálního humeru. Metodika: V bakalářské práci je vypracována kazuistika probanda s diagnózou zlomeniny proximálního humeru. U vybraného probanda jsem nejdříve odebrala anamnézu, poté jsem provedla vstupní kineziologické rozbor. Ten byl složený z aspekce, palpace, goniometrického a antropometrického měření, vyšetření pohybových stereotypů, speciálních testů na ramenní kloub - vyšetření joint play, vyšetření zkrácených struktur a hypermobility. Podle získaných údajů a nalezených patologiích jsem navrhla krátkodobý plán fyzioterapie. Intervence trvala 3 měsíce, pacient docházel na terapii dvakrát týdně první měsíc, druhý měsíc jednou za týden a třetí měsíc jednou za 14 dní. Délka jedné terapie trvala 45-60 min. Během cvičebních jednotek bylo využito následných fyzioterapeutických konceptů a metod (TMT, PIR, PNF, kinesiotejp, DNS, Brügger koncept, Spiraldynamik?, SM systém) k ošetření měkkých tkání, zvýšení rozsahu pohybu a svalové síly v ramenním kloubu. Při poslední návštěvě jsem provedla výstupní kineziologické vyšetření a zhodnotila efekt terapie. Dle výsledků výstupního kineziologického rozboru jsem navrhla dlouhodobý plán fyzioterapie. Výsledky: Zhodnocení proběhlo na základě porovnání výsledků ze vstupního a výstupního kineziologického rozboru. U pacienta došlo k zvýšení svalové síly a zvětšení rozsahu pohybu v pravém ramenním kloubu, odstranění kloubních blokád, zlepšení stereotypu abdukce v ramenním kloubu, snížení otoku PHK. Závěry: Vlivem intenzivní fyzioterapeutické intervence a edukace k autoterapii v domácím prostředí, a hlavně aktivnímu přístupu pacienta, se může pacient vrátit do své práce a k provozování běžných denních činností.

Abstract:

Introduction: In my bachelor's thesis, I dealt with physiotherapy for probands with a racture of the proximal part of the humerus. Theobjective: The aim of the work is to contribute to the solution of the choice of optimal physiotherapeutic procedures, concepts and methods in a selected client after a fracture in the proximal humerus. Methodology: The bachelor's thesis deals with a case report of a proband with a diagnosis of a proximal humerus fracture. I first took a history of a selected proband, then I performed an initial kinesiological analysis. It consisted of aspect, palpation, trigonometric and anthropometric measurements, examination of movement stereotypes, special tests for the shoulder joint - examination of joint play, examination of shortened structures and hypermobility. According to the obtained data and found pathologies, I proposed a short-term physiotherapy plan. The intervention lasted 3 months, the patient attended therapy twice a week for the first month, once a week for the second month and once every 14 days for the third month. The duration of one therapy lasted 45-60 min. Subsequent physiotherapeutic concepts and methods (TMT, PIR, PNF, kinesiotape, DNS, Brügger concept, Spiraldynamik?, SM system) were used during the exercise units to treat soft tissues, increase range of motion and muscle strength in the shoulder joint. At the last visit, I performed a final kinesiological examination and evaluated the effect of the therapy. According to the results of the final kinesiological analysis, I proposed a long-term physiotherapy plan. Results: The evaluation was based on a comparison of the results from the input and output kinesiological analysis. The patient had an increase in muscle strength and range of motion in the right shoulder joint, removal of joint blockages, improvement of the stereotype of abduction in the shoulder joint, reduction of swelling in the right upper limb. Conclusions: Due to intensive physiotherapeutic intervention and education forautotherapy in the home environment, and especially the active approach of the patient, the patient can return to his work and to the operation of normal daily activities.
