Accounting for inpatient case characteristics in cost models

Abstract:

Fair reimbursement of acute inpatient care is a key aspect of the transparent and effective functioning of a healthcare system, as well as of the high quality of provided care. However, the reimbursement mechanisms currently in place are not fully capable of responding to the high degree of individualisation of healthcare in a flexible manner. Therefore, this thesis explores the possibilities of applying commonly available characteristics of hospitalised patients in cost models in order to better explain cost variability. In addition to the currently utilised cost model, two different modelling approaches are applied to the reference hospitals’ data in the thesis to explain the impact of child age and discharge status on the level of costs in two medical domains. These approaches are then compared with each other in terms of their resulting parameters, as well as the impact on the allocation of fictional funds between the individual hospitals. The results presented suggest that the relationships between the studied factors and cost levels are complex and specific to the given clinical domain at hand, and that taking the studied characteristics into account cannot be both generalisable and simple to implement at the same time. The results of this thesis can therefore provide an analytical foundation for a deeper understanding of these relationships and further study of the implementation of routinely collected patient characteristics in reimbursement mechanisms.

Abstract:

Spravedlivá úhrada nákladů na akutní lůžkovou péči je klíčovým aspektem transparentního a efektivního fungování zdravotního systému a vysoké kvality poskytované zdravotní péče. V současnosti uplatňované úhradové mechanismy však nedokážou zcela flexibilně reagovat na vysokou míru individualizace zdravotní péče. Tato diplomová práce se proto zabývá možnostmi uplatnění běžně dostupných charakteristik hospitalizovaných pacientů v modelech nákladů za účelem lepšího vysvětlení nákladové variability. Kromě aktuálně platného nákladového modelu jsou na data referenčních nemocnic v práci aplikovány dva různé modelovací přístupy, které ve dvou medicínských oblastech objasňují vliv dětského věku a vliv způsobu ukončení hospitalizace na výší nákladů. Tyto přístupy jsou mezi sebou následně porovnány z hlediska jejich výsledných parametrů a také dopadu na přerozdělení fiktivních finančních prostředků mezi jednotlivými nemocnicemi. Prezentované výsledky naznačují, že vztahy mezi zkoumanými faktory a výši nákladů jsou komplexní a specifické pro danou klinickou problematiku a že zohlednění sledovaných charakteristik nemůže být současně plošné a implementačně jednoduché. Výsledky této práce tak mohou posloužit jako analytické podklady pro pochopení těchto vztahů a další uvažování nad implementací rutinně sbíraných charakteristik pacientů v úhradových mechanismech.
