Methods of evaluation of heel spurs in historical populations

Abstract:

Patní ostruha je považována za jednu z nejčastějších entezopatií na dolní končetině. Podle jejího výskytu ji dělíme na plantární a dorsální patní ostruhu. V klinické praxi je výskyt ostruhy spojován s bolestí paty. Studií o patní ostruze na kostech v suchém stavu nacházíme však velmi málo. Proto se jedním z cílů této práce stal stručný přehled o patní ostruze, faktorech ovlivňujících její vznik, hodnocení a klinické projevy ostruhy. Dále se práce zabývá návrhem a realizací vhodné metody pro analýzu patní ostruhy u historického kosterního materiálu. Pro tuto potřebu byl vybrán kosterní materiál ze tří historických období – pravěku, středověku a novověku. Celkově bylo hodnoceno 128 patních kostí od 95 jedinců. U jednotlivých kostí byla hodnocena přítomnost/nepřítomnost patní ostruhy a následně bylo provedeno její metrické vyhodnocení. Pro metriku patní ostruhy byly definovány modifikované rozměry podle Caroline Cermak a Sylvie Kirchengast (2015). Na základě výsledků popisné statistiky pozorujeme významný rozdíl výskytu dorsální patní ostruhy u pravěkého souboru a vyšší výskyt plantární patní ostruhy u starších jedinců.

Abstract:

The heel spur is is considered to one of the most common enthesopathy in the lower limb. Depending on its presence, we divided it into a plantar and a dorsal heel spur. In clinical practice, the occurrence of spurs has been associated with heel pain. However, we find very few studies on heel spurs on bones in a dry state. Therefore, the aim of this thesis is a brief overview of the heel spur, factors influencing its formation, an evaluation, and a clinical manifestation of the spur. Moreover, the thesis also deals with the design and implementation of an appropriate method for the analysis of heel spurs in historical skeletal material, which was selected from three historical periods – Prehistory, Middle Age and Modern age. Overall, 128 heel bones were evaluated from 95 subjects. The presence/absence of the heel spur was assessed on an individual bone basis followed by a metric evaluation. For heel spur metrics, modified dimensions were defined according to Caroline Cermak and Sylvie Kirchengast (2015). Based on the results of the descriptive statistics, we observe a significant difference in the incidence of dorsal heel spurs in the prehistoric population and a higher incidence of plantar heel spurs in the elderly.
