Diagnostic tool THINC-it for evaluating cognitive function in depressive disorders.

Abstract:

V této práci se zaměřujeme na posouzení kognitivních funkcí pomocí diagnostického nástroje THINC-it u vybraných pacientů s depresivní poruchou. S využitím dalších testových metod hodnotíme, do jaké míry úroveň kognitivních funkcí souvisí s doménami jako kvalita života, sociální a praktické fungování či pocit stigmatizace. V teoretické části se zabýváme nejprve samotnou depresivní poruchou, poté kognitivními funkcemi a kognitivním deficitem. Tyto oblasti popisujeme nejdříve obecně, poté specificky pro depresivní poruchu a pro období remise. Výzkumné části se zúčastnily osoby v aktuální depresivní epizodě, osoby v remisi a kontrolní skupina zdravých osob. Informace o úrovni kognitivních funkcí jsme získali nástrojem THINC-it, kvalita života byla posuzována škálou WHOQOL-BREF, fungování jedince v každodenních situacích bylo hodnoceno škálou GAF a pocit stigmatizace škálou ISMI. Získané výsledky ukazují, že kognitivní deficit je u jedinců s depresivní poruchou v remisi přítomen i po odeznění akutních depresivních symptomů a nadále ovlivňuje další oblasti jejich života. V mnohých testech kognitivních funkcí dosahují tito jedinci statisticky významných rozdílů oproti zbylým dvěma skupinám. Dále jsme zjistili, že úroveň kognitivních funkcí výrazně pozitivně koreluje s vnímáním celkové kvality života či schopností samostatně fungovat v každodenních situacích a výrazně negativně koreluje s pocity sebestigmatizace. Také v oblasti každodenního fungování byly mezi osobami v remisi a zdravými jedinci zjištěny další statisticky významné rozdíly. V některých testech kognitivních funkcí se však lidé v remisi od zdravých výrazně nelišili, stejně tak jako ve vnímání celkové kvality života. Tuto práci lze využít k poukázání na problematiku osob s depresivní poruchou v remisi a zejména na skutečnost, že i když v některých oblastech mohou tito jedinci dosahovat podobných výsledků jako zdravá populace, v jiných se tato skupina od zdravých výrazně liší. Výsledky mohou sloužit jako podklad pro další výzkumnou činnost v této oblasti a pro zdůraznění potřeby neuropsychologické rehabilitace i u osob v remisi.

Abstract:

In this work, we focus on the assessment of cognitive functions using THINC-it tool in selected patients with depressive disorder. Using other test methods, we evaluate the extent to which cognitive function levels are related to domains such as quality of life, social and practical functioning, or stigmatization. In the theoretical part, we deal with the depressive disorder itself, then with cognitive functions and cognitive deficit. These areas are described first generally, then specifically for depressive disorder and remission period. The research part included a group of people in the current depressive episode, a group in remission and a control group of healthy people. We obtained information about the level of cognitive functions with THINC-it tool, about the quality of life with the WHOQOL-BREF scale, the global functioning of the individual in everyday situations was assessed by the GAF scale and the stigmatization feeling by the ISMI scale. The results show that cognitive deficit is present even in individuals with the depressive disorder in remission and continue to affect other areas of their lives. In many cognitive tests, these individuals achieve statistically significant differences from the other two groups. Furthermore, we found that the level of cognitive functions correlates positively with the perception of the overall quality of life and the ability to function independently in everyday situations and correlates negatively with the feelings of self-stigma. Other statistically significant differences were also found in comparing patients in remission and healthy subjects in global functioning. However, in some cognitive tests, patients in remission did not differ significantly from the healthy subjects as well as in the perception of the overall quality of life. This work can be used to highlight the issue of patients with depressive disorder in remission and particularly the fact that although in some areas these individuals may achieve similar results as a healthy population, in others areas this group differs significantly from the healthy one. The results can serve as a basis for further research activities in this area and for emphasizing the need for neuropsychological rehabilitation also in patients in remission.
