Nurses´attitudes to terminally ill patients

Abstract:

Cíl: Cílem práce bylo zjistit, jaké postoje mají sestry k pacientům v terminálním stadiu, jak se postoje mohou během profesní praxe měnit a co ke změnám přispívá, ať pozitivně nebo negativně ve vztahu k pacientovi. Metodika: Výzkumný soubor tvořilo 50 dotázaných sester pracujících na odděleních, kde pečují o pacienty v terminálním stadiu. Dotazníkové šetření probíhalo od 12. - 23. 12. 2011 a bylo realizováno za pomoci anonymního dotazníkového šetření. Výsledky: Šetřením bylo zjištěno, že sestry chápou důležitost postojů, všechny zachovávají autonomii k pacientům, ale často odpovídají velice všeobecně, nejsou ovlivněny konkrétní situaci. Pozitivní vliv na péči má vyšší věk sestry, délka praxe a ztráta blízké osoby. Negativní vliv na postoje má nižší věk sestry, málo praktických zkušeností a opět ztráta blízké osoby. S eutanázií souhlasí většina dotázaných sester. Závěr: Terminální stadium a umírání je poslední fáze lidského života. V tomto období potřebuje člověk veliké zázemí, kde se cítí bezpečně a kde je postaráno o jeho potřeby. V této etapě je důležitý postoj sestry, protože postoje úzce souvisí s chováním sester, které mají být empatické, musí umět naslouchat a správně reagovat na všechny potřeby umírajících a jejich blízkých. Je velice žádoucí, aby se o postojích sester více mluvilo již na školách, kde se sestry na povolání připravují a dále je řada na zaměstnavatelích, aby postoje nepodceňovali a vytvořili takové pracovní podmínky, které budou sestry pozitivně ovlivňovat, protože to je klíč k úspěšné a motivující práci, která ovlivní paliativní péči o terminálně nemocné.

Abstract:

Target: Target of this thesis was to find out, what kind of attitude nurses keeps to their patients at the terminal stage, how can their stands be changing during their practice and what contributes to the changes, in a positive or negative way. Methodology: Investigated file was created by 50 respondents, which were nurses working at the departments, where is treated with patiens at the terminal stage. Investigation lasted from 12. - 23. 12. 2011 and was realized with anonymous questionnaire. Results: It was found out, that nurses do understand the importace of attitudes, all of them preserves autonomy to their patiens, but they did often answered in a very general way, they are not affected by any specific situations. Possitive influence to the health care is higher age of the nurse, length of her practice and a loss of close person. Negative influence comes with lower age, not enough experiences and a loss of close person again. All of them do agree with euthanasia. Conclusion: Terminal stage and dying is the last stage of human?s life. In this term person needs a big hinterland, where he feels save and where people take care of his needs. In this stage is very important the nurse?s attitude, because they should be emphatic, they should know, how to listen and react to the needs of dying person and his close family. Is very desirable to talk more about nurse?s attitude already at school, where are nurses studying and next turn is on employers, to make conditions, that will influenced nurses in a very positive way to succesfull and motivated work, that will improve the palliative care for patients in terminal stage of thein life.
