Body temperature monitoring of patients in intensive care

Abstract:

Monitorace tělesné teploty patří na odděleních intenzivní péče mezi základní sledované fyziologické funkce. Tělesná teplota může být příznakem infekčních i neinfekčních chorob, ale zároveň i samostatným problémem. Tělesná teplota přispívá také k rozeznání zlepšení či zhoršení stavu pacienta. V dnešní době je na trhu velký výběr přístrojů k monitoraci tělesné teploty. Na jednotkách intenzivní péče a anesteziologicko - resuscitačních odděleních dochází k častějšímu měření teploty. Způsoby monitorace mohou být invazivní a neinvazivní. V některých případech jsou použity invazivní metody k monitoraci centrální tělesné teploty nebo stačí jen monitorace povrchové tělesné teploty. Cílem práce je zjistit, jaké metody měření tělesné teploty se nejvíce používají na jednotkách intenzivní péče. Dalším cílem je zjistit, jaké jsou rozdíly v naměřených teplotách neinvazivními teploměry u pacientů v intenzivní péči. V praktické části jsem vybrala tři teploměry dostupné na odděleních, kde jsem měření prováděla. Jednalo se o teploměry pro neinvazivní monitoraci tělesné teploty. Vybrala jsem náhodně 30 pacientů v intenzivní péči, u kterých jsem provedla měření třemi vybranými teploměry. Měření probíhalo dle standardu oddělení 5x denně. Každý klient byl změřen 15krát. Naměřené hodnoty byly zaznamenány do tabulky v programu Excel. Následně došlo k zpracování a vyhodnocení výsledků. Žádná ze stanovených hypotéz se nepotvrdila. Ve 13 případech rozdíl mezi naměřenými teplotami přesáhl 0,5 C. Mezi bezkontaktním teploměrem a povrchovým teplotním čidlem byly naměřeny největší rozdíly

Abstract:

Body temperature monitoring is one of the basic monitored physiological functions in intensive care units. Body temperature can be a symptom of infectious and non-infectious diseases but also a separate problem. Body temperature also contributes to recognizing the patient's improvement or deterioration. Today, there is a large selection of body temperature monitoring devices on the market. In the intensive care units and anaesthesiology and resuscitation departments, the temperature is measured more frequently. Monitoring methods can be invasive and non-invasive. In some cases, invasive methods are used to monitor central body temperature, or surface body temperature monitoring is sufficient. The aim of this thesis is to find out which methods of measuring body temperature are most used in intensive care units. Another goal is to determine the differences in the measured temperatures by non-invasive thermometers in patients in intensive care. In the practical part, I selected three thermometers available in the departments where I performed the measurements. These were thermometers for non-invasive body temperature monitoring. I randomly selected 30 patients in intensive care and I measured with three selected thermometers. The measurement took place according to the standard of the department 5 times a day. Each client was measured 15 times. The measured values were recorded in a table in Excel. Subsequently, the results were processed and evaluated. None of the established hypotheses was confirmed. In 13 cases, the difference between the named temperatures exceeded 0,5 C. The biggest differences in the measured temperatures were between the non-contact thermometer and the surface temperature sensor.
