The role of nurses in preventive care of ischemic stroke in patients under 45 years of age

Abstract:

Bakalářská práce je zaměřena na možnosti preventivní činnosti z pozice specializované sestry u pacientů s ischemickou cévní mozkovou příhodou (iCMP) a rizikových skupin obyvatelstva. Je rozdělena na část teoretickou a analytickou. V části teoretické jsou popsány rizikové faktory iCMP, odlišnosti v různých věkových kategoriích, včetně diagnostiky, léčby a organizace péče o pacienty s iCMP v České republice. Dále jsou zde popsány aktuální možnosti primární a sekundární prevence. V hlavní části jsme vycházeli z analýzy dat, získaných z lékařské dokumentace pacientů našeho iktového centra, z dostupných českých a zahraničních studií, z rozhovorů se studenty chomutovského gymnázia a mladými pacienty našeho oddělení po prodělané iCMP. Na základě analýzy rizikových faktorů u 60 pacientů do 45 let, hospitalizovaných za posledních 5 let s iCMP na našem pracovišti, jsme vytvořili návrh metodiky práce sestry v cerebrovaskulární ambulanci se zaměřením na preventivní činnost u rizikových skupin obyvatel. Tato ambulance je neoddělitelnou součástí iktového centra, poskytující komplexní péči o pacienty s cerebrovaskulárním onemocněním. Návrh je jasně strukturován a obsahuje jednotlivé kroky, jak má sestra postupovat při edukaci pacienta v poradně. Při práci s množstvím materiálů bylo zjištěno, jak závažný problém představují rizikové faktory iCMP již ve skupině mladých dospělých ve věku do 45 let. České republice se systematickým preventivním opatřením věnuje jen málo pozornosti a akce zaměřené na prevenci jsou spíše ojedinělé. Dále bylo zjištěno, že úplně chybí možnost vzdělávání sester v problematice preventivní péče. Stěžejním výstupem práce je návrh konceptu metodiky práce sestry v cerebro-vaskulární poradně pro prevenci iCMP a vytvoření edukačních materiálů pro nejmladší populaci. Výsledky této práce budou využívány nejen v rámci našeho iktového centra v Chomutově, ale i na celostátní úrovni v rámci propagace důležitosti prevence iCMP a zdravého životního stylu.

Abstract:

The bachelor´s thesis is focused on the possibilities of the precautionary activity on the position of a specialised nurse regarding patients with ischemic stroke and risk groups of populations. It is divided into a theoretical and analytical part. The theoretical part describes risk factors of ischemic stroke, differences in various age categories, including diagnostics, treatment and organisation of care for patients with ischemic stroke in the Czech Republic. Further it describes current possibilities of primary and secondary precaution. In the main part, we started from the analysis of data acquired from the medical documentation of patients of our ictus centre, from available Czech and foreign studies, form interviews with pupils of the Grammar school in Chomutov and young patients of our department after suffered ischemic stroke. On the basis of analysis of risk factors in 60 patients aged up to 45 years hospital-ized in our workplace with ischemic stroke during the past 5 years, we have created a proposal of the methodology of nurse´s work in cerebrovascular ambulance with focus on precautionary activity in risk groups of populations. This ambulance is an inseparable part of the ictus centre providing comprehensive care for patients with cerebrovascular decease. The proposal is clearly structured and it includes individual steps, how nurses shall proceed when educating patients during counselling. In the course of work with large quantity of materials it was ascertained that the risk factors of ischemic stroke in a group of young adults aged up to 45 years represent already a serious problem. In the Czech Republic only a small amount of attention is dedicated to the systematic precautionary measures and the actions focused on prevention are rather isolated. Further it was ascertained that the possibility of education for nurses in the field of precautionary care is totally absent. The fundamental outcome is the proposal of a concept for methodology of nurse´s work in cerebrovascular counselling for precaution of ischemic stroke and creation of educational materials for the youngest population. The results of this work will be used not only in our ictus centre in Chomutov but also on a countrywide level within the scope of promotion of importance of precaution of ischemic stroke and healthy lifestyle.
