Cognitive dysfunction and cortisol level in first episode schizophrenia patients

Abstract:

Diplomová práce shrnuje a popisuje výsledky výzkumu věnujícího se deskripci vztahu kognitivního deficitu a kortizolémie u pacientů s první epizodou schizofrenie ve zkoumaném vzorku 23 mužských pacientů hospitalizovaných na PK LF MU a FN Brno. Pacienti byli vyšetřeni dexametazonovým supresním testem (před léčbou a na konci akutní léčby), škálou PANSS (před léčbou a na konci akutní léčby), a neuropsychologickými testy (Auditory verbal learning test, Test cesty, Verbal fluency test, Wisconsinský test třídění karet). Bylo zjištěno, že vyšší bazální kortizolémie souvisí s narušeným výkonem paměťových funkcí. Vztah mezi kortizolémií a exekutivní dysfunkcí je méně jasný. Ostatní měřené kognitivní funkce nejsou výší bazální kortizolémie přímo ovlivněny. Mezi postdexametazonovou kortizolémií a kognitivními funkcemi nebyla nalezena žádná souvislost. Nebyl nalezen žádný rozdíl mezi skupinou supresorů a nonsupresorů ve vztahu ke kognitivním funkcím. Z uvedených výsledků vyplývá, že bazální kortizolémie může ukázat na riziko deteriorace paměťových funkcí, její stanovení tedy může sloužit k detekci kandidátů pro případnou prokognitivní intervenci.

Abstract:

In this diploma thesis we discuss and summarize the results of research focused on the description of the relationship between cognitive dysfunction and cortisol levels in patients with first-episode schizophrenia. Our sample consisted of 23 male patients hospitalized at PK LF MU and FN Brno. Patients were administered these tests – dexamethasone suppression test (before treatment and at the end of acute treatment), PANSS (before treatment and at the end of acute treatment), and neuropsychological battery (Auditory verbal learning test, Trail making test, Verbal fluency test, Wisconsin card sorting test ). It was found that a higher basal cortisol levels were related to impaired performance in memory functions. Correlation between cortisol level and executive dysfunction is less explained. Other measured cognitive functions are not directly affected by basal levels of cortisol. There was no correlation found between cognitive function and postdexamethasone cortisolemia. No differences were found between the group of suppressors and nonsupressors in relation to cognitive functions. These data indicate that basal levels of cortisol can point out at the risk of deterioration of memory functions, in other words the determination of basal level of cortisol may serve to detect possible candidates for specific cognitive intervention.
