Abstract:

Úvod. Kongenitální ichtyózy jsou velmi heterogenní skupinou onemocnění projevující se suchou, hrubou a deskvamující kůží. U všech forem ichtyóz je v různém rozsahu porušena kožní bariéra. Vrozené ichtyózy jsou způsobeny mutacemi různých genů. Klinické projevy jednotlivých typů se v průběhu věku pacienta mění. V současné době je diagnostika kongenitálních ichtyóz postavena na molekulární analýze, která umožňuje i komplexní genetické poradenství a genetickou prevenci. Je vhodné pacienty dispenzarizovat ve specializovaných centrech, kde je možná spolupráce neonatologa, dětského dermatologa, lékařského genetika a dalších specialistů. Metody. V první části práce je popsán teoretický úvod současných znalostí o vlivu různých typů mutací na fenotyp kongenitálních ichtyóz. V druhé části práce byla analyzována data pacientů DKO PEK FN Brno. V souboru pacientů bylo zkoumáno zastoupení jednotlivých typů mutací. K analýze byla použita deskriptivní statistika a výsledky byly srovnány s literárními zdroji. Výsledky. K nejzávažnějším formám kongenitálních ichtyóz vedou recesivně zděděné mutace. V souboru bylo 129 pacientů, přičemž pacienti s autosomálně recesivními kongenitálními ichtyózami byli nejpočetnější skupinou. Zastoupení jednotlivých mutací odpovídá literárním zdrojům. Závěr. Ačkoliv jsou ichtyózy velmi heterogenní skupinou onemocnění s pestrým klinickým obrazem, metody molekulární biologie výrazně zpřesnily jejich diagnostiku, umožnily komplexní genetické poradenství v rodinách, kde se již ichtyóza vyskytla, a také genetickou prevenci. Vážně porušená epidermální bariéra představuje velké nároky na novorozeneckou péči, ale díky pokrokům v neonatologii se přežití novorozenců výrazně zlepšilo. Naproti tomu stále zůstávají velmi omezené možnosti lokální i systémové terapie. Kauzální terapie ichtyóz dosud neexistuje, je stále ve stádiu výzkumu. U závažných typů ichtyóz je nutná multioborová spolupráce, nejlépe ve specializovaných centrech s návazností na neonatologická pracoviště.

Abstract:

Background. Congenital ichthyoses are a very heterogeneous group of diseases manifested by dry, rough and scaling skin. In all forms of ichthyoses, the skin barrier is damaged to a certain degree. Congenital ichthyoses are caused by various gene mutations. Clinical manifestations of the individual types vary as the patient ages. Currently, the diagnosis of congenital ichthyoses is based on molecular analysis, which also allows a complete genetic counseling and genetic prevention. It is appropriate to dispensarize the patients in specialized medical centers, where the cooperation of a neonatologist, a pediatric dermatologist, a geneticist and other specialists is enabled. Methods. In the first part of the thesis a detailed review of how different mutations influence the phenotype of congenital ichthyosese is presented. In the next part, a data analysis of patients from the DKO PEK FN Brno was performed. The proportion of individual types of mutations was examined in the group of patients. Descriptive statistics were used for the analysis and the results were compared with literature sources. Results. Recessively inherited mutations lead to the most severe forms of congenital ichthyosis. There were 129 patients in the cohort, with patients with autosomal recessive congenital ichthyosis being the largest group. The representation of individual mutations corresponds to literature sources. Conclusions. Even though the ichthyoses are a very heterogeneous group of diseases with a very varied clinical picture, the methods of molecular biology have significantly increased their diagnostics, enabled complex genetic consultations in families in which the ichthyosis has already occurred, as well as genetic prevention. Severely damaged epidermal barrier represents huge demands on neonatal care, but because of the advantages in neonatology, the survival rate of newborns has increased significantly. On the other hand, the options of local and systemic therapy remain very limited. The causal therapy of ichthyoses does not exist yet, it is still under development. In severe types of ichthyoses, cooperation between multiple fields is necessary, ideally in specialized centers connected to neonatological sites.
