PCR diagnosis of invasive pulmonary aspergillosis

Abstract:

Tato bakalářská práce se zabývá problematikou PCR diagnostiky invazivní plicní aspergilózy, což je onemocnění představující významné riziko mortality pro imunokompromitované pacienty. Teoretická část práce popisuje rod Aspergillus jakožto původce tohoto onemocnění a možnosti jeho molekulární detekce zahrnující přímou detekci antigenů produkovaných těmito organismy a PCR detekci aspergilové DNA ze vzorku klinického materiálu. Důraz je kladen na faktory ovlivňující diagnostický a analytický výkon metody, jako jsou způsob extrakce DNA, primárně vyšetřovaný materiál či vybraná cílová oblast. V praktické části bylo provedeno testování senzitivity tří typů primerů se sekvencemi specifickými pro různé oblasti ribozomální RNA (5,8S, 28S a ITS1). Jedná se o primery pan-fungální, pan-aspergilové a primery specifické pro druh Aspergillus fumigatus, které byly otestovány dvěma typy polymeráz se sondami fungujícími na principu SYBR green a TaqMan. Detekce fungální DNA byla provedena metodou real-time PCR na vzorku druhu Aspergillus fumigatus. Tento vzorek byl testován v různých koncentracích. Pan-aspergilové primery, detekující všechny klinicky významné aspergily, byly navíc otestovány i na souboru vzorků různých druhů aspergilů. Potvrdili jsme, že primery umí zachytit všechny klinicky významné testované druhy (A. niger, A. flavus, A. terreus, A. clavatus a A. lentulus) včetně dalších méně významných druhů, kromě druhu Aspergillus nidulans. Délka a správnost výsledných PCR produktů byla ověřena gelovou elektroforézou. Nejvyšší senzitivity PCR detekce bylo dosaženo v případě pan-aspergilových primerů a nejnižší u pan-fungálních primerů. Detekce pomocí SYBR green měla vyšší senzitivitu, ale větší množství nespecifických produktů a validita detekce byla značně narušena tím, že pozitivitu vykazovaly i negativní kontroly. Polymeráza s TaqMan sondou měla naopak nižší senzitivitu detekce, ale méně nespecifických produktů a negativní kontroly byly negativní.

Abstract:

This bachelor's thesis deals with the issue of PCR diagnosis of invasive pulmonary aspergillosis, which is a disease presenting a significant risk of mortality for immunocompromised patients. The theoretical part of the work describes the genus Aspergillus as the causative agent of this disease and the possibilities of its molecular detection including direct detection of antigens produced by these organisms and PCR detection of Aspergillus DNA from a sample of clinical material. Emphasis is placed on factors affecting the diagnostic and analytical performance of the method, such as the method of DNA extraction, the primarily examined material or the selected target area. In the practical part, sensitivity testing of three types of primers with sequences specific for different regions of ribosomal RNA (5,8S, 28S and ITS1) was performed. These are pan-fungal, pan-Aspergillus primers and primers specific for Aspergillus fumigatus, which were tested by two types of polymerases with probes working on the principle of SYBR green and TaqMan. The detection of fungal DNA was performed by the real-time PCR method on a sample of Aspergillus fumigatus species. This sample was tested at various concentrations. Pan-Aspergillus primers, detecting all clinically significant aspergillus species, were additionally tested on a sample set of different Aspergillus species. We confirmed that the primers can capture all clinically important tested species (A. niger, A. flavus, A. terreus, A. clavatus and A. lentulus) including other less important species, except Aspergillus nidulans. The length and correctness of the resulting PCR products were verified by gel electrophoresis. The highest sensitivity of PCR detection was achieved in the case of pan-Aspergillus primers and the lowest in the case of pan-fungal primers. Detection using SYBR green had a higher sensitivity, but a larger amount of non-specific products, and the validity of the detection was greatly impaired by the fact that negative controls also showed positivity. In contrast, polymerase with a TaqMan probe had a lower sensitivity of detection, but fewer non-specific products and negative controls were negative.
