Physioterapy in pacient after stroke

Abstract:

Úvod: Cévní mozková příhoda se řadí k nejčastějším neurologickým poruchám, které vedou k chronickému postižení. Po CMP ztrácí pacienti v různé míře funkce motorické, senzitivní, senzorické a kognitivní, což vede k celkovému snížení kvality jejich života. Cíl práce: Cílem této bakalářské práce je přispět k řešení problematiky stavů po prodělané CMP pomocí fyzioterapeutických postupů, technik a metod. Metodika: Na základě zjištěných skutečností ze vstupního kineziologického rozboru byl stanoven krátkodobý plán fyzioterapie. Tento plán jsem následně s pacientem realizovala během následujících dvou měsíců spolupráce. Cvičební jednotky trvaly obvykle kolem 45 minut a byly při nich využity zejména tyto metody: facilitační metoda pomocí míčků, pasivní i aktivní pohyby, cvičení proti různým druhům odporu, cvičení s overballem, metoda PNF, cviky na různých labilních plochách. Fyzioterapeutická intervence byla ukončena provedením výstupního kineziologického rozboru, zhodnocením výsledků terapie a následně návrhem dlouhodobého plánu fyzioterapie. Výsledky: Podle výsledku výstupního kineziologického rozboru došlo ke zlepšení svalové síly a hybnosti na levostranných končetinách. Na dolní končetině se zvýšila síla svalů v oblasti kyčelního, kolenního i hlezenního kloubu. Na horní končetině se podařilo zvýšit svalovou sílu a rozsahy pohybů v kloubu ramenním a loketním. Zlepšení bylo zaznamenáno také v oblasti akra, kde je již pacient schopen provést alespoň některé úchopy nemocnou horní končetinou. Pokrok udělal pacient také ve zlepšení stability samostatného stoje a stereotypu chůze s pomůckou i bez pomůcky v interiéru i v exteriéru. Závěr: Za pomoci fyzioterapeutické intervence dosáhl pacient zlepšení kondice a svalové síly levostranných končetin a zlepšil se v chůzi i ve zdolávání schodů. Byl zaznamenám pokrok také v úchopové funkci ruky a oblasti sebeobsluhy.

Abstract:

Introduction: Stroke is one of the most common neurological disorders that lead to chronic disability. After CMP, patients lose motor, sensitive, sensory and cognitive functions to varying degrees, which leads to an overall reduction in their quality of life. Objectives: The aim of this bachelor's thesis is to contribute to the solution of the problem of conditions after undergoing CMP using physiotherapeutic procedures, techniques and methods. Methodology: Based on the findings from the initial kinesiology analysis, a short-term physiotherapy plan was established. I subsequently implemented this plan with the patient during the next two months of cooperation. The exercise units usually lasted around 45 minutes and the following methods were mainly used: facilitation method using balls, passive and active movements, exercises against various types of resistance, exercises with overball, PNF method, exercises on various unstable surfaces. Physiotherapy intervention was completed by performing an output kinesiological analysis, evaluating the results of the therapy and then proposing a long-term physiotherapy plan. Results: According to the results of the final kinesiology analysis, there was an increase in muscle strength and momentum on the left limbs. On the lower limb, the strength of the muscles in the area of the hip, knee and ankle joints increased. On the upper limb, it was possible to increase muscle strength and range of motion in the shoulder and elbow joints. Improvement was also noted in the acro region, where the patient is already able to perform at least some grips with the diseased upper limb. The patient also made progress in improving the stability of independent standing and the stereotype of walking with and without an aid indoors and outdoors. Conclusion: With the help of physiotherapy intervention, the patient achieved an improvement in the condition and muscle strength of the left limbs and improved in walking and climbing stairs. Progress was also noted in the grip function of the hand and the area of self-care.
