The selected psychological determinants in the context of recovery after PTCA

Abstract:

Tato diplomová práce se zaměřuje na vliv psychologických determinant zdraví na rekonvalescenci pacientů s ischemickou chorobou srdeční po zákroku PTCA. Sledovali jsme v době rekonvalescence jak fyzickou zdatnost pacientů, tak jejich psychickou pohodu, a také jsme se soustředili na změnu životního stylu pacientů. Sledovanými determinantami byly dispoziční optimismus, defenzivní pesimismus, health-efficacy (osobní vnímaná zdatnost spojená se zdravím) a internal health control (místo kontroly spojené se zdravím). Byly použity metody zjišťující dispoziční optimismus (LOT-R), defenzivní pesimismus (DPQ), health efficacy a internal health control (2 subtesty z dotazníku MHQ), fyzickou zdatnost a psychickou pohodu (dva subtesty z dotazníku SF-36) a námi vytvořený dotazník životního stylu. Uvedené dotazníky jsme administrovali v době hospitalizace a některé z nich také za 3 a za 6 měsiců po zákroku PTCA. Zjistili jsme, že pacienti s vysokou úrovní fyzické zdatnosti po celou dobu rekonvalescence měli nízkou míru defenzivního pesimismu, vysokou míru health-efficacy a internal health control. Stejné faktory se ukázaly být důležité i pro psychickou pohodu pacienta. Předpokládaný vliv dispozičního optimismu se překvapivě neprokázal jako statisticky významný. Statisticky významně vyšší míra zdravého životního stylu byla zjištěna u dispozičních pesimistů, zatímoco dispoziční optimisté vykazovali spíše méně zdravý životní styl. V průběhu rekonvalescence nedocházelo ke změně životního stylu pacientů.

Abstract:

This diploma thesis is focused on the influence of psychological determinants of health on recovery period of patients with ischemic heart disease after a PTCA procedure. During recovery period, we observed physical fitness of patients and their mental well-being and we also focused on the patients’ lifestyle change. The observed health determinants included dispositional optimism, defensive pessimism, health-efficacy and internal health control. Specific methods were used to find out dispositional optimism (LOT-R), defensive pessimism (DPQ), health efficacy and internal health control (2 subtests of the MHQ questionnaire), for physical fitness and mental well-being (two subtests of the SF-36), and a lifestyle questionnaire, we had prepared. The questionnaires were administered during hospitalization and some of them also three and six months after the PTCA procedure. We found out that patients with high level of physical fitness throughout the recovery had a low rate of defensive pessimism, a high rate of health-efficacy and internal health control. The same factors have been shown to be important for the patients’ psychological well-being. Expected impact of dispositional optimism hasn’t been surprisingly shown as statistically significant. A statistically significant higher rate of healthy lifestyle was found at dispositional pessimists, whereas optimists showed a rather less healthy lifestyle. The patients‘ lifestyle didn’t change during the recovery period.
