Physiotherapy for children affected by cerebral palsy

Abstract:

Cíl: Cílem mé bakalářské práce je přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u pacientů postižených dětskou Metodika: V bakalářské práci je zpracována kazuistika pacienta s diagnózou DMO. Spolupráce trvala necelý rok a celkem jsem provedl 18 návštěv. Využil jsem tyto koncepty: míčkovou facilitaci, pasivní pohyby, aktivní pohyby, cvičení s dopomocí i protiodporu, izometrická cvičení, senzomotorickou stimulaci, prvky z Vojtovy metody a konceptu ACT, Frenkelova cvičení a cvičení na aktivaci HSS. Výsledky: U pacienta došlo k zlepšení funkcí hrubé motoriky, z toho důvodu se posunul na stupnici GMFCS z III. stupně na II. stupeň a podle lokomočních stadií dle Vojty se posunul ze stadia 7 na stadium 8. Došlo k zlepšení stereotypu chůze, při níž pacient méně vychyluje trup, horní končetiny nejsou tak výrazně drženy v horních polohách, pánev je při chůzi stabilnější, dolní končetiny nejsou tolik flektovány v kolenou a kyčlích, došlo k prodloužení kroku. Chlapec je schopen chůze do schodů i ze schodů a chůze pozadu. Posílením trupového svalstva se chlapec přetočí ze zad na břicho a má lepší stabilitu trupu při sedu. Chlapec je schopen aktivovat hluboký stabilizační systém, aktivně překlopí pánev do retroverze a zaktivuje bránici.

Abstract:

Title: Physiotherapy for children affected by cerebral palsy Aims: The aim of my thesis is to contribute to solving problems of choice of optimal therapeutical methods, concepts and methods in case of patients affected by cerebral palsy. Methodology: The work includes a case of a patient diagnosed with cerebral palsy. The cooperation lasted less than a year and I did a total of 18 visits with him. I used the following concepts: Mick facilitation, passive movements, active movements, exercises with help and anti-resistance, isometric exercise, sensorimotor stimulation, elements of Vojta´s method and the koncept of Roswitha Brunkow, Frenkel exercises, to activate the HSS and back wolking. Results: Patients showed improvement in function of gross motor skills, hence shifted on a scale of GMFCS from the III. Instance to II. degree and locomotion by stages according to Vojta has moved from the seventh stage to the eight. An improvement in walking stereotype, in which patients less deflects the trunk, upper limbs are not as strongly held in the upper positions, the pelvis is stable when walking, legs are not so much flexed at the knees and hips, were extended step. The boy is able to walk up stairs and down stairs. Strengthening the trunk muscles, the boy rolls over from back to stomach and the abdomen has a better stability when sitting. The boy is able to activate the deep stabilizing system, actively tilted pelvis in retroversion and activates the diaphragm.
