Prognostic biomakers in patients with early stage breast cancer

Abstract:

Karcinom prsu patří nejenom mezi nejběžněji se vyskytující zhoubné novotvary u žen, ale představuje i nejčastější příčinu úmrtí způsobenou nádorovým onemocněním. Tato bakalářská práce pojednává o karcinomu prsu z hlediska jeho vzniku, rozšíření v populaci, rizikových faktorů, diagnostiky a terapie. Důraz je však kladen na prognostické biomarkery, které přispívají k odhadu rizika relapsu onemocnění u pacientek s časným klinickým stádiem karcinomu prsu. Pro odhad rizika recidivy jsou k dispozici komplexní modely, které zahrnují klinické, patologické nebo molekulární proměnné. Nadějným prognostickým biomarkerem se však jeví mikroRNA (miRNA). MiRNA jsou velmi stabilní krátké nekódující molekuly RNA, jejichž schopností je regulovat více než polovinu lidských genů. Významnou roli hrají především v procesech karcinogeneze. Cílem této práce bylo určit prognostické vlastnosti vybraných tkáňových miRNA (miR-34a, miR-210, miR-9 a miR-128a) ve vztahu k riziku relapsu onemocnění. Nalezli jsme signifikantní rozdíly hladin exprese tří miRNA (miR-34a, miR-210 a miR-128a) mezi skupinami pacientek s relapsem a bez relapsu do 5 let od diagnózy. Po validaci našich výsledků na větším souboru pacientek bude možné tyto miRNA považovat za slibné prognostické markery, a to u pacientek s hormonálně dependentním karcinomem prsu v časném klinickém stádiu tohoto onemocnění.

Abstract:

Breast cancer is not only the most commonly occurring malignant neoplasm in women, but is also the most common cause of death due to cancer. This thesis deals with breast cancer in terms of its formation, spread in population, risk factors, diagnosis and therapy. Emphasis is placed on prognostic biomarkers that help to estimate the risk of relapse in patients with early clinical stage of breast cancer. To estimate the risk of recurrence there are available complex models that include clinical, pathological and molecular variables. Promising prognostic biomarker, however, appears to be microRNA. miRNA are very stable short noncoding RNA molecules whose ability is to regulate more than half of human genes. An important role is especially played in carcinogenesis. The goal of this thesis was to determine the prognostic properties of selected tissue miRNAs (miR- 34a, miR-210, miR-9, miR-128a) in relation to the risk of relapse of the disease. We found significant differences in expression levels of three miRNAs (miR -34a, miR-210 and miR-128a) between the groups of patients with relapse and without relapse within 5 years since diagnosis. If our findings will be confirm on extended sample of patients it will be possible to consider these molecules as promising prognostic markers in patients with hormone positive breast cancer in early clinical stage of the disease.
