Calibration of colorectal cancer natural history simulation model

Abstract:

Tato diplomová práce se zabývá kalibrací simulačního modelu přirozeného průběhu onemocnění kolorektálním karcinomem. Markovský model je použit k simulaci průchodu kohorty pacientů skrz stavy definované v modelu. Cílem této práce je kalibrovat tento model na česká populační data za účelem potvrzení účinnosti kolorektálního screeningu v České republice. Za tímto účelem jsme vybrali několik cílů kalibrace, konkrétně se jedná o incidenci, mortalitu a rozdělení stádií incidence tohoto onemocnění a použili jsme Monte Carlo simulace pro měnění vstupních parametrů za účelem zajištění lepší shody s daty. Kalibrovaný model je poté použit k prokázání, že screening na kolorektální karcinom je efektivní prevencí mortality spojené s touto nemocí. Test na okultní krvácení každý rok od věku padesáti let a každé dva roky od 54 let věku je posuzován jako screenigová metoda. Redukce mortality specifické pro kolorektální karcinom byla spočítána, že se pohybuje okolo 10% až 37% v závislosti na účasti populace. Tímto byla prokázána možná efektivita screeningu v redukci mortality modelové populace na kolorektální karcinom.

Abstract:

This diploma thesis deals with the calibration of the natural history simulation model of colorectal cancer. We are using a cohort Markov model to simulate the movement of a cohort of patients among the model states. The goal of this thesis is to calibrate this model against Czech population data in order to prove the effectiveness of colorectal screening in Czech Republic. For this purpose we chose several targets of calibration, namely incidence, mortality and incidence stage distribution of the illness and used Monte Carlo simulation to vary input parameters so that we achieve the best fit. The calibrated model is then used to prove screening for colorectal cancer is an effective prevention of mortality associated with the disease. A faecal occult blood test every year from the 50 years of age and every second year from the 54 years of age is assessed as a screening method. We calculated the colorectal cancer specific mortality reduction to vary from around 10% to around 37% depending on the participation rate. This way a possible effectiveness of screening in reduction of colorectal cancer specific mortality within a modelled population was proven.
