The validation of the dataset for pressure lesions monitoring in relation to the nurses level of knowledge

Abstract:

Problematika dekubitálních lézí je stále nedoceněná, a to přesto, že se jedná o jednu z nejlépe preventabilních událostí v oblasti negativních dopadů přidělované péče, a představuje nejčastější nežádoucí jev spojený s poskytováním zdravotních služeb. V rámci projektových aktivit VES AZV MZČR byl připraven standardizovaný datový standard pro sledování prevalence dekubitálních lézí, který byl po dobu 4 let validován ve 3 fakultních nemocnicích a následně schválen MZ ČR pro plošné národní využití v Systému hlášení nežádoucích událostí (https://shnu.uzis.cz). Hlavním záměrem disertační práce pak bylo ověřit, jak se využívání datového standardu projeví v úrovni znalostí nelékařských zdravotnických pracovníků, kteří s ním pracovali, ve srovnání s kontrolní skupinou osob, které s validizovaným datovým setem neměli zkušenost. Zjištěno bylo, že celkově je úroveň znalostí u vybraného souboru respondentů – nelékařů v oblasti identifikace a klasifikace dekubitů srovnatelná v obou skupinách, ale personál pracující s datovým standardem byl schopen lepší diferenciální diagnostiky tlakových a vlhkých lézí. V ostatních sledovaných oblastech se úroveň znalostí nelišila, což lze považovat za významné zjištění, ale obecně korespondující se zahraničními zdroji. Vzhledem k faktu, že vývoj nových poznatků nejen v oblasti identifikace, prevence a léčby dekubitů je významně progresivní, je nezbytným předpokladem pro zkvalitnění péče kontinuální celoživotní vzdělávání.

Abstract:

The issue of pressure ulcers remains underestimated despite the fact that it is one of the best preventable events in the area of negative impacts of allocated care, and the most common adverse event associated with the provision of health services. Within the project activities of VES AZV MZČR a standardized data standard was prepared for monitoring the prevalence of pressure ulcers, which had been validated for 4 years in 3 faculty hospitals and subsequently approved by the Ministry of Health of the Czech Republic for the nationwide use in the System of Reporting Adverse Events (https://shnu.uzis.cz). The main aim of the dissertation was to verify how the use of the data standard will be reflected in the level of knowledge of non-medical healthcare workers who worked with it, compared to the control group of persons who had no experience with a validated data set. It was found that overall level of knowledge of the selected group of non-medical respondents regarding the pressure ulcers identification and classification is comparable in both groups, but the staff working with the data standard was able to better differential diagnosis of pressure and wet lesions. In other monitored areas the level of knowledge did not differ, which can be considered as a significant finding, but generally corresponding to foreign sources. Due to the fact that the development of new knowledge not only in the field of identification, prevention and treatment of pressure ulcers is significantly progressive, continuous lifelong learning is a necessary prerequisite for improving the quality of care.
