Determination of a control group of healthy donors by examination stimulated platelet aggregation by light transmission aggregometry

Abstract:

Úvod: Krevní destičky hrají klíčovou roli v procesu zástavy krvácení v takzvané primární hemostáze při tvorbě primární destičkové zátky. Při této činnosti není důležitý jen jejich počet, ale také jejich správná funkce. Mezi základní funkce krevních destiček patří adheze a agregace. Vyšetření adhezivity krevních destiček je pouze hrubě orientační, proto se většinou nepoužívá. Pro zjištění funkčnosti destiček se využívají například přístroj PFA-100 pro kvantitativní měření primárni hemostázy, nebo přístroje na principu impedanční agregometrie a světelné transmisní agregometrie pro zjištění agregačních schopností krevních destiček po stimulaci jednotlivými induktory. Teoretická část bakalářské práce pojednává o fyziologické hemostáze a o hlavních složkách hemostázy se zaměřením na krevní destičky a vyšetření jejich funkce. Praktická část se zabývá vyšetřením funkce krevních destiček, kvantitativním vyšetřením primární hemostázy na přístroji PFA-100 a především vyšetřením stimulované agregace krevních destiček na agregometru Apact 4004 pracujícím na principu světelné transmisní agregometrie. Cíl studie: V této práci bylo hlavním cílem určení fyziologického referenčního rozmezí agregačních křivek jednotlivých induktorů u zdravých dárců při vyšetření stimulované agregace krevních destiček metodou světelné transmisní agregometrie na přístroji Apact 4004. Výsledky: Fyziologické referenční rozmezí finálních agregací jsou pro ADP 76,40 % ? 91,00 %, kolagen 72,44 % ? 91,77 %, epinefrin 78,57 % ? 93,25 %, kyselinu arachidonovou 83,24 % ? 93,82 % a ristocetin 84,89 % ? 97,49 %. Referenční rozmezí lag fáze u kolagenu je 66,38 s ? 219,74 s. Závěr: Referenční rozmezí vyšla velmi úzká. Pro použití referenčních rozmezí k diagnostice v praxi by bylo vhodné mít soubor o velikosti alespoň 60, nejlépe 120 lidí. Abstract:

Introduction: Platelets have a key role in stopping the bleeding in the primary hemostasis in the formation of primary platelet plug. In this activity is not important only their quantity but also their proper function. The basic functions include the platelet adhesion and aggregation. The examination of platelets adhesivity is only for approximately orientation, so it is not usually used in practice diagnosis. To determine the functionality of the plates are used for example PFA-100 device for the quantitative measurement of primary hemostasis, or devices on the principle of impedance aggregometry and light transmission aggregometry for detecting aggregation ability of platelets after stimulation with different inductors. The theoretical part of this thesis is about the physiological hemostasis and about the major components of hemostasis focusing on platelets and the platelets function testing. The practical part of this thesis is about examination of platelet function, the quantitative examination of primary haemostasis by PFA-100 and about examination of stimulated platelet aggregation by light transmission aggregometry on the device Apact 4004. Objective: In this thesis is the main objective the determination of the physiological reference aggregation curves ranges of various inductors in healthy donors group by stimulated platelet aggregation by light transmission aggregometry on the device Apact 4004. Results: Physiological reference ranges of final aggregations are for ADP 76,40 % - 91,00 %, for collagen 72,44 % - 91,77 %, for epinephrine 78,57 % - 93,25 % for arachidonic acid 83,24 %, - 93,82 % and for ristocetine 84,89 % - 97,49 %. Reference range of lag phase for collagen is 66,38 s ? 219,74 s. Conclusion: The reference ranges are very close. To use the reference ranges for diagnosis in practice it would be desirable to have a control group of size at least 60, better 120 people.
