Delirious condition in intensive medicine

Abstract:

Delirium v podmínkách intenzivní medicíny dosahuje až 62 % a u pacientů na umělé plicní ventilaci až 82 %. V oboru intenzivní medicíny je prevalence deliria spojena se zvýšenou morbiditou a mortalitou, horším dlouhodobým klinickým výsledkem a vyššími ekonomickými náklady. Delirium je však pod diagnostikované pro nepoužívání standardizovaných diagnostických nástrojů. Teoretická část práce ve své první části popisuje etiologii a výskyt deliria na JIP, prevenci a léčbu těchto stavů. Druhá kapitola rozebírá hodnotící nástroje pro detekci deliria na jednotkách intenzivní péče. Třetí kapitola popisuje strategii léčby a prevenci u rizikové skupiny - seniorů v podmínkách intenzivní medicíny. Poslední kapitola se věnuje potřebám pacienta v intenzivní péči a týmové spolupráci v péči o nemocné. Empirická část práce byla zaměřena na úroveň znalostí sester v Moravskoslezském kraji o deliriu a klinické praxi v podmínkách intenzivní péče kvantitativním výzkumem metodou dotazníkového šetření. Dalším výzkumným vzorkem byli hospitalizovaní pacienti 1. a 3.den hospitalizace na ICU a byli hodnoceni standardizovaným testem CAM-ICU pro detekci deliria. Výsledky byly následně ověřeny pomocí vybraných statistických metod a na jejich základě byly stanoveny hypotézy. Výzkumným šetřením byla zjištěna úroveň znalostí sester o delirantních stavech a klinické praxi a především byla zjištěna incidence deliria v podmínkách intenzivní péče.

Abstract:

Delirium in intensive care conditions reaches up to 62% and in patients on artificial lung ventilation up to 82%. In the field of intensive care, the prevalence of delirium is associated with increased morbidity and mortality, worse long-term clinical outcomes and higher economic costs. However, delirium is underdiagnosed for not using standardized diagnostic tools. The theoretical part of the work in its first part describes the etiology and occurrence of delirium in the ICU, prevention and treatment of these conditions. The second chapter discusses evaluation tools for the detection of delirium in intensive care units. The third chapter describes the strategy of treatment and prevention in the risk group - seniors in intensive care. The last chapter deals with the needs of the patient in intensive care and teamwork in patient care. The empirical part of the work was focused on the level of knowledge of nurses in the Moravian - Silesian region about delirium and clinical practice in the conditions of intensive care by quantitative research using the method of questionnaire survey. Another research sample was hospitalized patients on days 1 and 3 of hospitalization at the ICU and were evaluated by the standardized CAM-ICU test for delirium. The results were then verified using selected statistical methods and based on them hypotheses were established. The research survey found the level of knowledge of nurses about delirium and clinical practice and, above all, the incidence of delirium in intensive care.
