Use of telerehabilitation in respiratory muscle training in patients with diseases of the respiratory system

Abstract:

posledním desetiletí došlo k významnému rozšíření využívání online vedené rehabilitace u různých diagnóz, mimo jiné i u diagnóz spojených s respiračním onemocněním. Tato práce se zaměřuje na využití telerehabilitace, jakožto formy plicní rehabilitace, u pacientů po transplantaci plic a u pacientů s astma bronchiale. Telerehabilitace je schopna obsáhnout všechny prvky plicní rehabilitace, recentní studie také ukazují, že má srovnatelné výsledky s klasickou plicní rehabilitací. Jelikož se jedná o vedení a poskytování rehabilitace na dálku, mezi hlavní výhody telerehabilitace patří zvýšení dostupnosti rehabilitace pacientům, pro které je dosažení klasické plicní rehabilitace velmi obtížné, nebo nemožné. Praktická část práce zkoumá efekt tréninku nádechových svalů formou telerehabilitace na jejich sílu u pacientů s onemocněním respiračního systému. Výzkumný soubor tvořilo celkem 23 probandů - 12 pacientů po oboustranné transplantaci plic a 11 pacientů s astma bronchiale. Pacienti byli rozděleni do dvou skupin. Obě skupiny absolvovaly stejný 6týdenní rehabilitační program, který zahrnoval trénink nádechových svalů pomocí trenažéru Airofit Airocure a cvičební jednotku zaměřenou na odporový trénink. Intervenční skupina byla supervizována formou telerehabilitace, zatímco kontrolní skupina byla ponechána bez supervize. Výsledky prokázaly vliv rehabilitačního programu na zvýšení síly nádechových svalů (MIP) a zlepšení tolerance zátěže (1-Minute Sit- to- Stand Test) bez rozdílu u obou skupin pacientů. Ani u jedné skupiny nedošlo ke zmírnění percepce dušnosti (dotazník mMRC). Nebyl zaznamenán rozdíl zlepšení v žádném sledovaném parametru mezi pacienty v intervenční a kontrolní skupině.

Abstract:

In the last decade, there has been a significant expansion in the use of online guided rehabilitation for various diagnoses, including those associated with respiratory conditions. This study focuses on the utilization of telerehabilitation as a form of pulmonary rehabilitation in patients after lung transplantation and in patients with bronchial asthma. Telerehabilitation is capable of incorporating all elements of pulmonary rehabilitation, and recent studies indicate that it has comparable outcomes to conventional pulmonary rehabilitation. As it involves remote management and facilitation of rehabilitation, the main benefits of telerehabilitation include increased accessibility of rehabilitation for patients who find accessing conventional pulmonary rehabilitation difficult or impossible. The practical part of the work examines the effect of inspiratory muscle training through telerehabilitation on their strength in patients with respiratory system diseases. The research population consisted of a total of 23 subjects - 12 patients post bilateral lung transplantation and 11 patients with bronchial asthma. Patients were divided into two groups. Both groups underwent the same 6-week rehabilitation program, which included inspiratory muscle training using the Airofit Airocure trainer and an exercise unit focused on resistance training. The intervention group was supervised via telerehabilitation, while the control group was left unsupervised. The results proved the effect of the rehabilitation program on increasing inspiratory muscle strength (MIP) and improving exercise tolerance (1-Minute Sit-to-Stand Test) without any difference in both groups of patients. Neither group experienced a reduction in the perception of breathlessness (mMRC questionnaire). There was no difference in improvement in any of the parameters studied between patients in the intervention and control groups.
