Možnost využití hiporehabilitace u dětí s DMO, přístup z hlediska ergoterapeuta

Possibility of using hiporehabilitation for children with CP, approach from the view of occupational therapist.

Abstract:

Diagnóza dětské mozkové obrny (DMO) patří mezi nejčastěji indikované skupiny v oblasti hiporehabilitace. I přes to chybí metodiky a standardy postupů hiporehabilitační intervence nejen u této diagnózy. Právě ergoterapeut zde pak může mít rozhodující vliv na komplexní využití a propojení všech složek terapie v různých funkčních rovinách rozvoje pacienta. Neboť zde dochází nejen ke korekci patologií a udržení zdraví, ale také k zapojení jedince do smysluplných činností za účelem zajistit zlepšení funkcí. Facilitujeme i učení a rozvoj hry. Teoretická část je tedy věnována vymezení DMO, jejímu rozdělení, projevům a možnosem terapeutických postupů. Dále se zaměřuje na základní definování a rozdělení zooterapie se zaměřením na hiporehabilitaci a možnosti jejího využití u dětí s DMO. Přibližuje indikační skupiny, kontraindikace a účinky hiporehabilitace na zdraví klientů. Praktická část práce je věnována kazuistice klienta s diagnózou dětské mozkové obrny, sestavení ergoterapeutického plánu pro tohoto klienta, následný popis terapeutické intervence včetně vstupního a výstupního vyšetření modifikovaného pro potřeby hiporehabilitace. Dále vyhodnocení účinku a efektu terapie. Na základě studia teoretických poznatků a praktické aplikace byla navrhnuta další terapeutická opatření a doporučení. Výsledky potvrdily příznivý vliv hiporehabilitace a její možnosti komplexního využití v rámci ucelené rehabilitace u dětí s diagnózou DMO. Terapie měla nezanedbatelný podíl na rozvoji držení těla, úpravě svalového tonu, zvětšení hybnosti, rozvoji hrubé i jemné motoriky a samostatnosti. V diskusi je poukázáno na možnost použít získané informace a sepsané postupy jako inspiraci právě pro tvorbu chybějících metodik a dalších komplexních plánů využitelných u hiporehabilitace poskytované ergoterapeuty i fyzioterapeuty.

Abstract:

The diagnosis of cerebral palsy (CP) is one of the most frequently indicated groups in the field of hipporehabilitation. Despite this, the methodologies and standards of procedures for hipporehabilitation interventions are missing not only for this diagnosis.The occupational therapist can have crucial influence on the complex use and interconnection of all parts of therapy in various functional levels of patient development. Not only is there a correction of pathologies and maintenance of health but also involvement of the individual in meaningful activities for the purpose to ensure the improvement of functions. We also support teaching and development of game playability. The theoretical part is therefore devoted to the definition of CP, its division according to character of the disorder, manifestations and possibilities of therapeutic methods. It is also focused on basic defining and dividing of zootherapy by methods with focus on hipporehabilitation and possibilities of its use in children with CP. It describes indication groups, contraindications and effects of hipporehabilitation on the health of clients. The practical part is devoted to a case report of a client with diagnosis of cerebral palsy, assembling an occupational therapy plan for this client, a subsequent description of the therapeutic intervention including initial and final examination modified for the needs of hipporehabilitation. Furthermore it contains the evaluation of impacts and effects of the therapy. Therapeutical measures and recommendations were suggested based on the study of theoretical knowledge and practical use. The results confirmed the positive effect of hipporehabilitation and its possibilities of complex use within integrated rehabilitation in children diagnosed with CP. The therapy had significant effect in the development of posture, adjustment of muscle tone, increased mobility, development of gross and fine motor skills and independence. The discussion points out the possibility of using gained information and written procedures as an inspiration for the creation of missing methodologies and other complex plans applicable in hipporehabilitation provided by occupational therapist and physiotherapists.
