Explicit and implicit processes leading to the realisation of the necessity of therapy in people with alcohol dependence.

Abstract:

Anotace: Předkládaná práce se zabývá zmapováním okolností epizod bažení v době abstinence a faktory, udržujícími v době abstinence hrozbu relapsu. Cílem práce je vytvoření modelu, který integruje kognitivně behaviorální model relapsu a motivační model ambivalentního vztahu k alkoholu a aplikace tohoto integrativního modelu na vysvětlení epizod bažení v době abstinence. K dalším cílům patří popsání vzájemných vztahů, mezi udržujícími faktory závislosti, které předpokládá kognitivně behaviorální a motivační přístup. Designem výzkumu je případová studie. Design výzkumu umožnil dlouhodobou spolupráci mezi výzkumníkem a účastníkem výzkumu. Ke sběru dat byly použity eseje účastníka výzkumu, polostrukturované rozhovory, a záznamy o zkoumaném jevu, které prováděl účastník výzkumu. Výzkum probíhal v období mezi prvním týdnem až osmým měsícem po ukončení účastníkovy léčby. Výsledky ukázaly, že faktory předpokládané kognitivně behaviorálním a motivačním modelem se mohou při podněcování bažení projevovat současně, ale také nezávisle na sobě. Bažení může být spuštěno aktivací podmíněných pravidel a automatických myšlenek v souvislosti s pocity křivdy, frustrace, rezignace a sebelítosti. Jakmile je spuštěno, závisí na míře závazku abstinovat. Role závislého při aktivaci bažení spočívá v jeho sebedemoralizující samomluvě. Na udržování bažení se abstinující závislý podílí tehdy, pokud vědomě zvažuje a rozvíjí možnost návratu k pití. Ve vztahu k obecnějším faktorům, udržujícím hrozbu relapsu, se práce zabývá souvislostí ambivalence a systému dysfunkčních předpokladů.

Abstract:

Abstract: The thesis deals with the topic of mapping the circumstances in connection to the episodes of craving in the period of abstinence, as well as with the factors which control the threat of relapse in this period. The goal of this thesis is to set a pattern which integrates both the cognitive behavioral pattern of the relapse and the motivational pattern of the ambivalent relationship to alcohol, and subsequent application of this integrative pattern to an explanation of episodes of craving in the period of abstinence. Another goal of this thesis is to describe mutual relations between the factors controlling the addiction which assume a cognitive behavioral and motivational approach. The way the research was carried out was designing a case study. This design of the research enabled a long-term cooperation between the researcher and the research participant. Essays of the research participant, semi-structured interviews and participant´s notes about the phenomenon which was the object of the research were used for the data collection. The research was carried out during the period of the first week and the eighth month after the treatment of the participant w as discontinued. The results showed that the factors assumed by the cognitive behavioral and motivational pattern can manifest themselves both simultaneously as well as independently during the process of instigating the craving. Craving can be set off by activation of maladaptive assumptions and automatic thoughts in relation to feelings of grievance, frustration, resignation and self-pity. As soon as it is set off, it depends on the degree of commitment to abstain. The role of the addict then lies in his or hers self demoralizing soliloquy. If the addict deliberately ponders the possibility of starting drinking again, then he or she contributes to the preservation of his or hers craving. The thesis also discusses the connections between ambivalence and the system of dysfunctional beliefs in relation to more general factors contributing to the threat of relapse.
