The causes of PICC´s dysfunction

Abstract:

Tato bakalářská práce se zabývá příčinami dysfunkce PICC katétru. Je rozdělena na teoretickou a praktickou část. V teoretické části je stručná historie žilních vstupů, jejich dělení se zaměřením na z periferie zaváděný centrální katétr. U PICC katétru jsou rozebrány indikace, kontraindikace, správný postup při zavádění, možné komplikace při jeho zavádění a používaní, které vedou k jeho dysfunkci a základy ošetřování tohoto katétru. V praktické části je zpracován soubor 40 pacientů, u kterých byl zaveden PICC katétr a následně byli sledováni až do vytažení katétru nebo do úmrtí pacienta s cílem zjistit průměr žil na obou pažích, komplikace při používání PICC katétru, které vedly k jeho dysfunkci. Průměrná délka zavedení PICC byla u respondentů 43,8 dne. Nejčastějším důvodem k zavedení byla u 23 pacientů (57,5 %) aplikace chemoterapie. Průměrná délka zavedení PICC u pacienta s nádorovým onemocněním byla 50,2 dne a u nenádorového onemocnění 31,1 dne. Nejširší žílou na paži v daném souboru byla u sledovaného souboru respondentů levá vena basilica s průměrem 3,89 mm. V průběhu sledování se vyskytla komplikace u 10 pacientů (4krát se jednalo o komplikaci trombotickou, 3krát infekční a ve 3 případech se jednalo o nechtěné vytažení pacientem). Většina pacientů s komplikací (9/10) byla onkologicky nemocná. Komplikace se vyskytly v 5,7 případech na 1000 dnů se zavedeným katétrem.

Abstract:

This bachelor thesis deals with the causes of PICC catheter dysfunction. It is divided into the theoretic and practical sections. Brief history of venous access, venous access classification with the focus on PICC form the content of the theoretical part. Indications, contraindications proper technique of PICC insertion, possible complications during the insertion procedure and its use leading to catheter dysfunction as well as the basic principles of PICC care are commented in this section. A cohort of 40 patients who underwent PICC insertion is presented in the practical part of the thesis. All the patients were followed until the catheter removal or until their death. The vein diameter was measured on both upper limbs (arms) before the PICC insertion. The complications during the PICC use leading to its dysfunction were recorded. The mean lenght of PICC insertion was 43,8 days. The most frequent indication for catheter insertion was the administration of chemotherapeutic agents (23 patients, 57,5%). The mean lenght of PICC insertion was different between oncologic and non-oncologic patients with the mean duration of catheter insertion 50,2 and 31,1 days respectively. The basilic vein of the left upper limb proved to be the widest vein in our cohort, with the mean diameter of 3,89mm. Ten patients had PICC related complication during the follow-up (4-times thrombotic complication, 3-times infectious complication and 3-times unintentional removal). Majority of the patients with complication (9 out of 10) were oncologic patients. The incidence of complications was 5,7 cases per 1000 catheter days.
