Abstract:

Cílem napsání diplomové práce je rozbor problému financování zdravotnictví v České republice a nalezení východisek, které povedou ke zvýšení kvality lékařské péče. Jelikož financování čehokoliv, co je hrazeno alespoň částečně ze státního rozpočtu, je politicky velice citlivé téma, skýtá mnoho variant, které vedou k vyřešení problému financování zdravotnictví. Řešení však mohou být správná nebo špatná a až budoucnost nám ukáže, zda varianta, kterou zvolíme, bude ta správná. Stejně jako v jiných státech, tak i v České republice, má financování zdravotnictví svojí historii. Od částečně solidárního systému, přes systém, kdy veškerá zdravotní péče byla hrazena prostřednictví státu, až po současnost, kdy se musíme zamyslet nad tím, jak bude zdravotnictví vypadat v následujících letech. Každá změna, která se v minulosti udála, byla vyvolána určitou situací. Ta poslední, která byla vyvolána i nízkou efektivností ve financování zdravotnictví, se dotkne pacientů v příštím roce. To je pouze začátek reformy, ty důležitější kroky jsou nachystány na další roky. Jedná se zejména o změny týkající se zdravotních pojišťoven a poskytovatelů zdravotní péče. Při řešení problému financování zdravotnictví jsem se inspiroval i způsobem financování zdravotnictví v zahraničí a aplikuji některé z charakteristických rysů do reformy v České republice. Proto důležitým zdrojem informací byly publikace o problematice financování zdravotnictví v zahraničí. V závěru práce hodnotím stav financování zdravotnictví v České republice a nastiňuji východiska, která by mohla vést k větší efektivitě díky vyššímu podílu spoluúčasti pacientů. Tato spoluúčast však musí být sociálně únosná. Vzhledem k současnému vysokému hospodářském růstu je ta nejvyšší doba začít konat.

Abstract:

The objective for writing dissertation is to analyze the problem of Czech Republic health service funding and finding outlet that will lead to medical care quality increase. Because funding of whatever, what is at least partially, from national budget is politically very sensitive topic, it provides a lot of alternatives, which leads to solve the problem of funding health service. But solutions could be correct or incorrect and only future show us if chosen alternative is the right one. As well as in other countries the history of health service funding in Czech Republic has own history. From partially fellow-feeling system, through system where whole health care was covered by state budget, till present when we must take a think how the health care service will be like in next years. Each change occurred in past was invoked by specific situation. The last one was invoked by low effectiveness of health care funding and will strike the patients next year. But this is only beginning of the reform and more important steps are prepared for years after 2008. The changes will strike health insurance companies and providers of health care service. While solving the problem of health care funding, I have inspired by method of health care funding in foreign countries and apply some of characteristic features to Czech Republic reform. That is why publications which occupies with problems of health service funding in foreign countries were very important source of information. At the end of dissertation I sum up condition of health care funding in Czech Republic and outline way which could lead to higher effectiveness due to higher rate of patients’ participation. But the participation must be socially tolerable. With regard to current high economic growth is high time to do something.
