Childbirth from the Perspective of Social Sciences: An Ethnography of Two Czech Maternity Hospitals with a Special Focus on their (Organizational) Cultures

Abstract:

Předkládaná práce je etnografií organizačních kultur dvou českých porodnic. Prezentuje zjištění z dvacetiměsíčního etnografického výzkumu, který jsem uskutečnila v období od roku 2004 do roku 2007 ve dvou porodnicích. Ty se nacházejí ve městech, jež v práci nazývám Javovem a Konžany. V práci usiluji o hlubší porozumění organizačnímu kontextu, v němž se v českém prostředí odehrává naprostá většina porodů. Primárně vycházím ze sociálních studií lidské reprodukce, konkrétně ze sociologie a antropologie porodu. Významně čerpám i z organizačních studií, z nichž se nechávám inspirovat konceptem organizační kultury. Cílem mé práce je na základě provedeného výzkumu porozumět organizačním kulturám zkoumaných porodnic a pochopit, jak ovlivňují poskytovanou porodní péči a tím i zdraví žen, jež v daných porodnicích rodí. Práce se soustředí na perspektivu zaměstnanců porodnic, které v ní vystupují jako klíčoví nositelé organizačních kultur. Organizační kultury analyzuji na třech různých úrovních. Na mikro úrovni se zabývám interakcemi mezi personálem a jeho klientelou. Zkoumám jak personál nahlíží na rodičky a dle jakých rysů rozlišuje jejich různé kategorie. Na meso úrovni studuji vliv procesů, které se odehrávají v rámci určité profese. Konkrétně se věnuji profesi porodních asistentek a porodníků-gynekologů. Na makro úrovni se soustředím na společenské procesy a jevy, jež přesahují konkrétní kontext obou porodnic a ovlivňují nejen jej, ale celý porodní „systém“. Jde zejména o vliv konzumerismu a komercionalizace v oblasti porodních (zdravotních) služeb.

Abstract:

This work is an ethnography of two Czech maternity hospitals and their organizational cultures. It is the result of twenty months of ethnographic research that I conducted in the towns of Javov and Konzany in the Czech Republic from 2004 to 2007. In this work I gain a deep and nuanced understanding of the organizational context within which the vast majority of births take place. My work primarily builds upon social studies of human reproduction, in particular the sociology and anthropology of childbirth, while it also draws on organizational studies, from which I borrow the concept of organizational culture. The goal of this work is to understand the organizational cultures of these two maternity hospitals, and to comprehend how their organizational cultures influence birth care and, by extension, the health of birthing women. The study focuses on the perspective of the maternity hospitals’ employees whom I perceive as main bearers of the organizational cultures. I analyze organizational cultures at three distinct levels. On a micro level I focus on interactions among maternity hospital employees and their clients. I explore how the staff view and approach birthing women and the characteristics by which they categorize them. On a meso level I study how professional processes of midwives and obstetrician-gynecologists affect the organizational cultures. On a macro level I concentrate on social processes and phenomena that transcend the specific maternity hospital context and affect the whole Czech birthing “system”. In particular I concentrate on consumerism and commercialization in birth (health) care.
