Validation of tools evaluating procedural children´s pain

Abstract:

Cíl: Cílem diplomové práce bylo zjistit psychometrické vlastnosti měřícího nástroje NIPS (Neonatal/Infant Pain Scale) a měřícího nástroje FLACC (Face, Legs, Activity, Cry, Consolability scale) hodnotících intenzitu bolesti u dětí do jednoho roku věku při zavádění periferního žilního katétru. Metodika: Výzkumný soubor tvořilo 116 kojenců, u kterých byla hodnocena dvěma dětskými sestrami intenzita bolesti při zavádění PŽK s využitím měřícího nástroje FLACC a NIPS. K ověření stanovených hypotéz byla zvolena metoda validační studie. Výzkum byl realizován v období od dubna do října 2016 na dětském oddělení Slezské nemocnice v Opavě a Nemocnice ve Frýdku-Místku. Výsledky: Zjistilo se, že zvolené měřící nástroje FLACC a NIPS jsou reliabilní (rozdíl naměřených výsledných bodových hodnot škálou FLACC i NIPS mezi sestrou 1 a sestrou 2 nebyl statisticky významný), validní (hodnocení škály FLACC i NIPS v kategorii bolest/bez bolesti odpovídalo objektivně naměřeným fyziologickým funkcím v kategorii nad normu/norma), specifické a senzitivní (za účasti tepové frekvence a tělesné teploty nebyly zjištěny rozdíly mezi specificitou a senzitivitou škály NIPS a FLACC). Dále byla zjištěna 100% shoda mezi hodnotiteli bolesti jak u škály FLACC, tak u škály NIPS. Také byl zjištěn vyšší průměrný čas (o 1,2 minuty) potřebný na vyhodnocení škály NIPS oproti škály FLACC. Dále nebyl zjištěn rozdíl ve výsledné intenzitě bolesti měřené škálou NIPS a škálou FLACC (98,2% shoda). Závěr: Hodnocení intenzity procedurální bolesti je nezbytné pro její prevenci a léčbu. V praxi by měla sestra pracovat s ověřenými měřícími nástroji. K vhodným měřícím nástrojům pro hodnocení intenzity procedurální bolesti u kojenců patří ověřený hodnotící nástroj NIPS a FLACC.

Abstract:

Objective: The objective of the thesis was to find out the psychometric properties of measuring tool NIPS (Neonatal/Infant Pain Scale) and measuring tool FLACC (Face, Legs, Activity, Cry, Consolability scale) which evaluate the intensity of pain with infants aged up to one year, while introducing a peripheral venous catheter. Methodology: The research pool was formed of 116 infants, whose intensity of pain was evaluated by two nurses while introducing a PVC using the measuring tools FLACC and NIPS. To verify the above stated hypotheses a validation study method was applied. The research was carried out in the period between April and October 2016 at the children's wards in the Silesian hospital (Slezská nemocnice) in Opava and a hospital in Frýdek-Místek. Results: It was found that the measuring tools FLACC and NIPS are reliable (the difference between point results measured using the FLACC as well as NIPS scales between nurse 1 and nurse 2 was not statistically important), valid (the evaluation of scale FLACC as well as NIPS in the category pain/no pain corresponded to objectively measured physiological functions in the category above normu/norma), specific and sensitive (with the participation of pulse frequency and body temperature no differences between the specificity and sensitivity of the NIPS and FLACC scales were found). Further, there was 100% common accord with the pain evaluators using both the FLACC and NIPS scales. Also, longer average time (by 1.2 minutes) was needed for the evaluation of the NIPS scale compared to the FLACC scale. The difference between the resulting pain intensity measured by the NIPS scale and the FLACC scale (98.2% common accord) was not found either. Conclusion: The evaluation of the procedural pain intensity is necessary for its prevention and treatment. Nurses in their practice should work with verified measuring tools. Measuring tools NIPS and FLACC rank among a verified evaluation tool suitable for the evaluation of procedural pain intensity.
