The influence of rTSM and ECT on the cognitive deficit of depressed patients

Abstract:

U depresí představuje kognitivní výkon pacientů a jeho poškození značné omezení v jejich běžných životních funkcích. Bylo zjištěno, že i u depresivních poruch dochází ke vzniku kognitivního deficitu, který se po zlepšení nálady zlepšuje. Kognitivní výkon pacientů ovlivňuje řada faktorů. Mezi ně patří i způsob zvolené léčby. Repetitivní transkraniální magnetická stimulace je poměrně novou metodou v léčbě deprese. Studie zabývající se touto otázkou ukazují, že rTMS může kognitivní funkce zlepšovat. Studie sledující vliv elektrokonvulzivní terapie na kognitivní funkce ukazují, že sice z krátkodobého hlediska je kognitivní deficit přítomen, ale z dlouhodobého hlediska není její účinek na kognitivní funkce tak výrazný. V této práci jsme zkoumali vliv rTMS a ECT na kognitivní funkce. Před vlastním zahájením a po ukončení aplikace rTMS a ECT byla hodnocena závažnost depresivních příznaků .Na začátku a po ukončení obou terapií bylo v stejném časovém odstupu od prvního vyšetření u obou skupin provedeno neuropsychologické vyšetření kognitivních funkcí. Kognitivní deficit depresivních pacientů se zlepšil ve skupině léčené rTMS. Tato metoda ukázala určitý prokognitivní účinek. Ve skupině léčené ECT byl kognitivní deficit přítomen i po ukončení terapie, byl ale kvalitativně odlišný.

Abstract:

Patients with depression and deteriorating cognitive functions feature severe restrictions in their daily functionality. Research has demonstrated that cognitive deficit is present even in depressed patients. This deficit is improved when the depression improves. Various factors influence patients' cognitive performance. One of them is the method of treatment. Repetitive transcranial magnetic stimulation is a new method in the treatment of depression. Papers addressing this issue show that rTMS can improve cognitive functionality. Papers referring influence on patients' cognitive functions of electroconvulsive therapy has shown that cognitive deficit may be present in the short term, but its affects on cognitive functions are not so pronounced in the long term. We investigated the influence of rTMS and ECT on the cognitive functions. The magnitude of the symptoms of depression was evaluated in the beginning and at the edn of treatment. Neuropsychological assessments were performed in the beginning and after the end of treatments for each group with the same time interval between assessments. Cognitive deficit was decreased in the rTMS group after treatment. rTMS showed a certain procognitive effect. Cognitive deficit was present in the ECT group even after the treatment, but was qualitative different.
