Immunological aspects of organ transplantation in humans

Abstract:

Transplantace orgánů jsou indikovány v případech, kdy se farmakologická léčba u pacientů jeví jako nedostatečná nebo neúspěšná. Spektrum náhrady tkání a orgánů sahá od transfuze červených krvinek přes transplantaci kostní dřeně až po transplantace celých orgánů jako jsou ledviny, játra, srdce, slinivka, plíce a v současné době na experimentální úrovní také transplantace končetin a obličeje. Úspěch transplantace závisí na kvalitě dárcovského orgánu, kvalitě chirurgického výkonu a imunosupresivní terapii. Největším nedořešeným problémem transplantací je odhojovací reakce, která je indukována v těle příjemce. Ústřední úlohu v iniciaci poškození štěpu sehrávají rozdíly mezi HLA (lidský leukocytový antigen) antigeny dárce a příjemce. Cílem této práce bude formou přehledového článku shrnout současné poznatky o mechanismech imunologické tolerance při orgánových transplantacích a pokusit se nastínit perspektivní směry, kterými by mohl být veden další výzkum.

Abstract:

Organ transplantation is indicated in cases where pharmacological treatment appears to be insufficient or unsuccessful in the matter of the patients. The spectrum of tissue and organ replacement goes from transfusion of red blood cells through bone marrow transplantation to whole organ transplantation such as kidneys, liver, heart, pancreas, lungs, and currently on an experimental level limb and face transplantation as well. The success of transplantation depends on the quality of the donor organ, the quality of the surgical procedure, and immunosuppressive therapy. The biggest unsolved problem of transplants is the regrowth reaction that is induced inside the recipient's body. The main role in the initiation of graft damage is the differences between the HLA (Human Leukocyte Antigen) antigens of the donor and the recipient play a role. The aim of this work will be to summarize current knowledge about mechanisms of immunological tolerance in organ transplants in the form of an overview article and try to outline prospective directions in which further research could be conducted.
