Management of invasive hemodynamic monitoring in intensive care

Abstract:

Diplomová práce je zaměřena na invazivní monitoraci hemodynamiky na pracovištích intenzivní péče. V teoretické části je popsán koncept managementu v ošetřovatelství, zmíněn historický mezník vzniku jednotek intenzivní péče, objasňována hemodynamická nestabilitaa, její patofyziologické důsledky. Dále diagnostické možnosti - klinické vyšetření a především monitorování hemodynamické parametrů. Ty jsou rozděleny na klasicky využívané a na rozšířený monitoring. Význam stanovených hodnot vzhledem k posouzení funkce kardiovaskulárního systému a základní principy jednotlivých metod používaných k jejich stanovení. V poslední kapitole je objasněna důležitost ošetřovatelské péče o pacienta s invazivní monitorací hemodynamiky. Empirická část diplomové práce zkoumá pomocí dotazníkového šetření, jehož se zúčastnilo 112 respondentů z pracovišť intenzivní péče, úroveň znalostí v této problematice v souvislosti s nejvyšším dosaženým vzděláním, specializačním vzdělání v oboru ARIP a typem nemocničního zařízení. Dále jsou analyzovány způsoby zaškolení, spokojenost se vzděláváním a kompetence sester v oblasti invazivní monitorace hemodynamiky v souvislosti s typem nemocničního zařízení. Výstupem této práce bylo zhodnotit zapojení sester do managementu invazivní monitorace hemodynamiky, tedy jejich kompetence. Možnosti zlepšení úrovně znalostí a to jak teoretické i praktické, které je možno získat jak kvalitním zaškolováním pracovníků, tak jejich permanentním vzděláváním. Znalosti jsou primárním předpokladem v kvalitním poskytování specializované ošetřovatelské péče a úzce souvisí s managementem invazivní monitorace hemodynamiky, tedy kompetencemi v této oblasti.

Abstract:

The diploma thesis is focused on invasive monitoring of hemodynamics in intensive care workplaces. The theoretical part describes the concept of management in nursing, the historical milestone of the formation of intensive care units, the hemodynamic instability, its pathophysiological consequences. Further diagnostic possibilities - clinical examination and especially monitoring of hemodynamic parameters. These are divided into classically used and extended monitoring. The significance of the established values in relation to the assessment of cardiovascular function and the basic principles of the individual methods used to determine them. The last chapter explains the importance of nursing care for patients with invasive hemodynamic monitoring. The empirical part of the diploma thesis is investigated by means of a questionnaire survey there were 112 respondents from intensive care centers, the level of knowledge in this issue in relation to the highest education achieved, the ARIP specialization education and the type of hospital facility. In addition, there are analyzed ways of training, satisfaction with education and competence of nurses in the area of invasive hemodynamic monitoring in connection with the type of hospital facility. The output of this work was to evaluate the involvement of nurses in management of invasive monitoring of hemodynamics, ie their competence. Possibilities to improve the level of knowledge, both theoretical and practical, which can be obtained both by quality training of workers and by their permanent education. Knowledge is a prerequisite for high-quality provision of specialized nursing care and is closely related to management of invasive hemodynamic monitoring, ie competencies in this field.
