Patient Care with ECCO2R therapy

Abstract:

Diplomová práce se zabývá péčí o pacienta s ECCO2R terapií neboli s mimotělním odstraněním CO2. Práce je rozdělena na část teoretickou a empirickou. Teoretická část se věnuje z počátku historii prvního používání mimotělního odstranění CO2 a fyziologii výměny plynů, dále se zaměřuje na systém provedení a fungování terapie, poté na indikace a kontraindikace, komplikace, které mohou nastat při terapii EC-CO2R a na antikoagulační léčbu. Další téma, které je v teoretické části zmíněno jsou druhy terapeutické léčby ECCO2R, která se využívá, a je dostupná. Posledním zmiňujícím tématem jsou specifika ošetřovatelské péče u pacienta s mimotělním odstraněním CO2. Empirická část je založena na kvalitativním výzkumu, který je zpracován formou případové studie, ve které jsou popsány kazuistiky pacientů, kteří metodu léčby ECCO2R podstoupili na jednotce intenzivní péče. Data jsou porovnávána s různými zahraničními studiemi za účelem zjištění funkčnosti odstranění CO2 touto metodou a zdali se během léčby vyskytly nějaké komplikace, které by souviseli s léčbou. Dalším stanoveným cílem bylo zjistit, zda dojde během terapie ke snížení hodnot pCO2 a následně tak možnosti úpravy ventilačního režimu. Na základě zjištěných dat bylo vytvořeno doporučení pro praxi.

Abstract:

The thesis deals with the care of a patient with ECCO2R therapy or extracorporeal CO2 removal. The thesis is divided into theoretical and empirical part. The theoretical part is initially devoted to the history of the first use of extracorporeal CO2 removal and the physiology of gas exchange, then it focuses on the system of execution and functioning of the therapy, then on indications and contraindications, complications that may occur during ECCO2R therapy and on anticoagulation therapy. Another topic that is mentioned in the theoretical part is the types of ECCO2R therapeutic treatment that is used and available. The last topic mentioned is the specifics of nursing care in the patient with extracorporeal CO2 removal. The empirical part is based on qualitative research, which is presented in the form of a case study, in which case reports of patients who underwent ECCO2R treatment in the intensive care unit are described. The data are compared with various international studies to determine the functionality of CO2 removal by this method and whether any treatment related complications occurred during the treatment. Another stated aim was to determine whether there would be a reduction in pCO2 values during therapy and consequently the possibility of adjusting the ventilatory regime. Based on the findings, a recommendation for practice was made.
